二尖瓣返流患者的心功能评价:超声心动图和MRI的对照研究(1)

     [摘要] 目的 采用超声心动图Simpson方程法量化评价二尖瓣返流患者的心功能参数,并与MRI所测结果进行对照。方法 36例患者(男11例;女25例)纳入研究。使用MRI测量左室功能参数,并与超声结果比较。心功能参数的比较采用配对t检验和Pearson相关分析,并采用Bland-Altman analysis评价两种检查方法的一致性。结果Simpson方程法测得左心室功能参数(EDV,ESV,SV和EF)分别为145.4±38.9ml、66.1±15.9ml、79.5±31.1ml和54.8±10.1%;MRI对左心室功能参数的评测结果分别为159.5±60.3ml,77.8±33.1ml,81.7±39.7ml,和48.8±9.7%。统计分析表明超声低估了左室舒张和收缩末期容积参数(EDV:p=0.038;ESV:p=0.040),而EF值则被高估(p=0.043)。Pearson相关分析表明其相关性较一般(r=0.45- 0.79)。结论 MRI可准确评价二尖瓣反流患者的心功能参数,从而为临床治疗及预后评价提供科学依据。

    Quantitative Assessing Left Ventricular Function in Patients with Mitral Regurgitation: Magnetic Resonance Imaging: Comparison with Echocardiography

    [Abstract] Objective To evaluate whether the MRI can assess the left ventricular function with high accuracy when compare the Simpson results with echocardiography. Methods 36 patients with mitral regurgitation underwent echocardiography and MRI examination for assessing the global ventricular function. Ventricular parameters including end-diastolic and end-systolic volume, stroke volume, and ejection fraction were measured with dedicated cardiac analysis software on MRI and echocardiography. Left ventricular function parameters of two methods were compared by using paired t test and Person's correlation, and the agreement between MRI and echocardiography was assessed with Bland-Altman analysis.
