[摘要] 目的 比较两种不同原理(离子交换高效液相色谱,亲和高效液相色谱)的糖化血红蛋白A1c(HbA1c)测定方法，以评价不同方法检测HbA1c结果间的偏倚。方法 依据美国临床实验室标准化委员会(NCCLS)EP9-A2，每天取8份临床病人新鲜血，分别使用两种方法，按1、2、3、4、5、6、7、8、8、7、6、5、4、3、2、1的顺序对样本进行测定，重复5天；计算每个样本测定的均值( 和 )，以对作散点图，以观察两种方法测定结果之间的相关性；以( - )对 做偏移图，以观察两种方法测定结果间的偏倚；利用去除离群点的数据，计算两个方法间的直线性回归方程，并对两种方法测定病人标本糖化血红蛋白结果进行预期偏倚估计。结果 同一份病人样本的测定结果，方法间以及方法内的差异均不大。两种方法间的线性回归方程为y=1.032x-0.212,r=0.9916(r2=0.9833)，在4.8%-16.0%范围内，两种方法测定糖化血红蛋白相对偏倚在-1.25%至1.86%间。结论 在常见的生理及病理范围内，两种方法测定的结果间表现出了有很好的可比性。

Comparison of Two Methods for Measurement of Hemoglobin A1c in Human Blood

[Abstract] Objective To compare two methods with different principles for measurement of hemoglobin A1c in human blood and Estimate the bias between the results of two methods. Mehtods Based on National Committee for Clinical Laboratory Standards EP9-A2, 8 fresh patients whole blood samples were collected everyday and measured by two methods in the sequence of 1、2、3、4、5、6、7、8、8、7、6、5、4、3、2、1 and the evaluation procedure was repeated for 5 days. Average of every sample results was calculated, and scatter plot ofversuswas made to check the linear relationship between X and Y throughout the measured range. Bias plot of( - )versus was made to check the difference between two methods. Data excluded outlier was used for linear regression. Results The linear equation is y=1.032x-0.212, r=0.9916(r2=0.9833).The relative bias between two methods is from-1.25% to 1.86% when HbA1c level falls in the range of 4.8% to 16.0%. Conclusion At usual physiological and pathological range, the bias between results measured by two methods shows a good comparability.

目前国内临床实验室用于糖化血红蛋白(GHB)检测的方法有很多，根据分析原理，大致可以分为两类：一是基于糖化血红蛋白与非糖化的血红蛋白的电荷不同，如离子交换色谱、电泳和等电聚集方法；二是基于血红蛋白上糖化基团的结构特点，如亲和色谱和免疫方法。本文对两种不同原理的糖化血红蛋白测定方法进行了比对，以了解不同方法测定结果间的可比性，并对他们之间的偏倚进行科学的评估。
