[摘要] 目的 通过目标人群的真正参与,找出存在的健康问题,制定切实可行的传播策略和健康教育的督导与评估方案,改变不科学、不健康的生活方式，以达到提高目标人群健康知识知晓率和健康行为形成率的目的。方法 抽取辖区内260名糖尿病患者(超过45周岁)，随机将他们分为两组(干预组与对照组)，干预组实行参与式健康教育管理，对照组实行慢性病常规管理，两组管理前均进行糖尿病相关知识、自我管理能力及态度问卷调查，管理一年后两组再次进行相同问卷调查，并将结果进行评价分析。结果 干预组医学知识、态度形成及各项卫生行为形成状况显著提高,而对照组无显著差别。结论 参与式健康教育对普及社区居民糖尿病及并发症的医学知识效果显著，应加大在社区卫生服务机构推广普及应用。

[Abstract] Objective It is to find out the health problems make practical propagation tactics and the evaluation issue of health education, change the unscientific unhealthy life style, improve the target population awareness rate of health knowledge and health behavior formation rate by target populations being really involved. Methods Extracted 260 diabetic patients (more than 45 years old) were selected, randomly divided them into two groups (intervention group and control group).The patients were implemented participatory health education. The control group was implemented routine management of chronic disease. The two groups are related with knowledge, self management ability and attitude questions. One year later, the two groups were surveyed with the same question. The results were analyzed and evaluated. Results The results of medical knowledge, attitude formation and various health behavior formation conditions in the intervention group improved significantly. Conclusion Participatory health education on popularization of community diabetes and complications of medical knowledge effect, should be approved the community health series of institutions.
