【摘要】目的:对比分析不同时期重度子痫前期的临床资料,探讨早发型重度子痫前期的特点。方法:回顾分析2009年1月至2011年1月大连市妇产医院收治的126例重度子痫前期患者的临床资料,按重度子痫前期发病孕周分为≤32周的早发组30例,>32周的晚发组96例。比较两组孕妇基本情况、生育史、部分临床项目及辅助检查结果、围生儿情况。结果:两组中首发症状、ALB、HCT、并发症发生率差异新生儿体重有统计学意义(P<0.05)。重度子痫前期组HCT增高、ALB明显降低，尤早发组组；但24小时尿蛋白和总蛋白无显著性差异。早发组新生儿体重低，Apgar评分低，死胎引产比例高，预后差。首发症状以血压高为主，其次是水肿，尿蛋白最少。发生胎盘早剥、肝肾损害等比例多。住院治疗时间短多在3到10天。而Hgb 、SBP、 DBP 、MAP、PROT、CHOL、TG、Fb、心输出量、外周阻力、血黏度、Aptt、SG比较均无明显差异。结论:重度子痫前期在不同孕周的妊娠结局有差异。早发型重度子痫前期可视为一种特殊类型的子痫前期。
Aspecialpre-eclampsia : early onset severe pre-eclampsia.

【Abstract】Objective:To study on the characteristics of early onset severe pre-eclampsia by comparing the data observed at different gestational weeks.Methods:One hundred and twenty-six cases meeting the criteria of severe pre-eclampsia were enrolled in this study.Patients were divided into 3 groups: early onset group(n=30) with those onset ≤ 32 weeks of gestation and late onset group(n=96)with onset>32weeks of gestation and normal pregnents(n=30). To compare their basic symptom and signs ,results of checks,outcome of pregnancy.Result ① The incidence ofearly onset group is23.8%. ②The first symptom inearly onset groupis hypertension(18/30)、edema(15/30)、proteinuria(24/30 ). ③ The incidences of placental abruption and liver and kidney damage are high in early onset group . ④The time in hospital often is 3-10days. ⑤The early onset group has lowerApgar score、 lowerneonatalbody weight、more stillbirth of induction of labor、poor prognosis. ⑥ The HCT in early onset group is higner andALB in early onset groupis lower .There isSignificant differencebetween early onset group and late onset group. Thereare not Significant differenceof Hgb 、SBP、 DBP 、MAP、PROT、CHOL、TG、Fb、cardiac output、peripheral vascular resistance、Blood viscosity、Aptt、SG. Conclusion The pregnancy outcome ofdifferent gestational week in severe pre-eclampsia is differenct. Early onset severe pre-eclampsia is a specialpre-eclampsia .
