瘦素与2型糖尿病\肥胖及胰岛素抵抗的关系(1)

【摘要】 目的 研究2型糖尿病(T2DM)和肥胖患者血清瘦素(LEP)水平及其与胰岛素抵抗的关系。方法 采用病例对照研究,T2DM伴有肥胖组(DO)42例,T2DM不伴肥胖组(NDO)42例,单纯性肥胖组(OB)39例,正常对照组(NC)28例。检测了4组研究对象血脂、血糖、空腹胰岛素(FINS)、LEP水平,用HOMA模型公式计算胰岛素抵抗指数(HOMA IR)。结果 DO组、OB组的LEP水平明显高于NDO组及NC组,差异有统计学意义(P<0.05)。LEP与性别、体重指数(BMI)和体脂百分含量(BFC)呈显著正相关(P<0.01),与腰围(WC)和FINS呈正相关(P<0.05);男女性别之间差异有统计学意义。结论 血清LEP水平与2型糖尿病、胰岛素抵抗无直接相关性,而与肥胖和性别相关。

【Abstract】 Objective To investigate the relationship between the level of serum leptin and insulin resistance in patients with Type 2 Diabetes Mellitus or obesity.Methods A case-control study was carried out to investigate serum lipids,fasting glucose,fasting insulin and leptin in 42 case of type 2 diabetes with obesity(DO),42 cases of type 2 diabetes without obesity(NDO),39 cases of simple Obesity(OB)and 28 cases of normal control(NC).Insulin resistance index was calculated with HOMA model formula.Results The level of serum leptin of group OB and DO were significantly higher than those of group NDO and NC.Correlation analysis showed that the serum leptin level was positively correlated with sex,BMI and BFC(P<0 ......
