【摘要】目的：探讨高血压患者的社区强化管理模式。方法：从辖区筛选出年龄在36～65岁之间患高血压者200例，随机分为对照组、实验组各100例。对照组实施常规社区管理，而实验组执行一套经过规划的管理流程。2年后对2组患者的相关情况进行评价。结果：实验组与对照组比较，前者的服药率、治疗率、控制率、高血压知晓率有显著提高。结论：采取社区强化管理是控制高血压有效的方法。

    Objective: to strengthen community-based hypertension management model.
