西药房药品发放差错72例原因分析及对策

摘要：目的探讨西药房药品发放差错的原因以及对策。方法对我院西药房2012年1月~2013年1月的72例药品发放差错记录进行回顾，分别对发放错误的药品种类以及发放错误的原因进行总结分析。结果72例发放错误的药品种类主要包括抗生素类药物，中药针剂，心血管疾病药物，外伤用药，消化内科药以及神经内科用药；发药差错的原因主要包括处方书写差错，药品名称误解，剂量差错，给药剂型不符，药品外观相似造成差错和分发已过期或变质药物。结论医院应建立严格的规章管理制度，医师和药师应加强业务学习和责任心，以降低药品发放差错，提高服务质量。

Abstract：ObjectiveTo discuss the reason and measure of medicines distributed error in pharmacy. Methods72 cases of medicines distributed error in our pharmacy from Jan. 2012 to 2013 were analyzed retrospectively.
