探讨白内障超声乳化术中后囊破裂的原因和处理技巧(1)
摘要：目的 探讨白内障超声乳化术中后囊破裂的产生的原因和处理技巧。方法 选取我院眼科自2011年6月～2014年6月收治的白内障超声乳化吸出及后房型人工晶体术1200眼作为研究对象，对其中发生后囊破裂情形、原因、处理技巧进行分析研究。结果 本组病例在手术中发生后囊破裂的患者68眼，其中后发生在发生于水分离时3只眼，超声乳化时为45眼，发生在注吸皮质时15眼，发生在植入人工晶体时5眼。术中恰当处置后全部植入人工晶体，随访3～6个月视力在0.5以上的患者52眼，视力在0.2以上者16眼，均未发生严重的并发症。结论 白内障超声乳化手术中并发后囊破裂不可避免，技术娴熟尽可能减少，一旦发生及时发现并采取正确的处理是提高手术效果的关键。

Cause Analysis and Treatment Techniques of Capsular Rupture During Cataract Phacoemulsification

Abstract：Objective To investigate the causes and treatment techniquesproduce capsular rupture during cataract phacoemulsification. Methods Selected the Department of ophthalmology in our hospital from 2011 June～2014 year in June from phacoemulsification and posterior chamber intraocular lens implantation in 1200 eyes as the research object， carried on the analysis to the one occurrence of posterior capsular rupture situation， causes， treatment techniques. Results In this group of patients in the surgical capsule rupture occurred in 68 eyes after the patients， after which occurred in the water separation occurred in 3 eyes phacoemulsification in 45 eyes， occurred in the injection and suction cortex in 15 eyes， 5 eyes occurred in the implantation of artificial crystal. Proper disposal of all operation after the implantation of artificial lens， the follow-up of 3-6 months， the vision in more than 0.5 patients 52 eyes，visual acuity at more than 0.2 in 16 eyes， there were no serious complications. Conclusion Phacoemulsification surgery complicated with posterior capsule rupture inevitable， skilled reduce as far as possible， once happen， timely find and take the correct treatment is the key to improve the effect of operation.

白内障超声乳化及人工晶体植入手术因具有切口小、损伤小、术后散光小、视力恢复好等优点已广泛推崇，而发生后囊破损是该手术最常见的并发症之一。后囊破裂原因是多方面的，与患者自身的状况、手术者操作熟练程度等诸多因素有关，因此在行超声乳化手术的过程中术者一定要做好十足的把握，谨慎的对待手术过程中的任何一个细节，一旦发生后囊破裂的并发症应该采取合理的方法进行处理，尽量避免发生更严重的并发症。由于近年来白内障的发病率逐年上升，因此采用超声乳化手术治疗的患者也逐渐增多，发生晶状体后囊破裂的自然相应增加，探讨后囊破裂的原因和处理方法已成为眼科界关注的话题之一。本文就对白内障超声乳化术中后囊破裂的产生原因和临床的处理技巧进行研究，现将研究结果报告如下。
1 资料与方法
1.1一般资料 选取我院眼科2011年6月～2014年6月收治的白内障行超声乳化手术1200眼作为研究对象，其中男613眼，女587眼，年龄32～92岁，平均年龄(62.4±2.1)岁，随访时间3～6个月。其中老年性白内障患者1096眼，所占比例为91.33%；并发性白内障患者56例，所占比例为4.67%；外伤性白内障患者38例，所占比例为3.17%；先天性白内障患者7例，所占比例为0.58%，其他3例(如青光眼、玻切术后等)，所占比例为0.25%；按晶状体核硬度进行分级：其中Ⅰ～Ⅱ级核246眼，占20.5%，Ⅲ～Ⅳ级核占954眼，占79.5%。, 百拇医药(尹成建)
