Mammotome联合超声引导精准乳腺病灶切除疗效观察(1)
摘要：目的 探讨Mammotome系统在彩色超声引导下精准乳腺病灶切除及活检中的应用疗效。方法 新疆医科大学第一附属医院2015年1月～2016年6月，收治124例乳腺病灶患者共144个病灶在彩色超声引导下行Mammotome切除术，回顾性分析并评价其在乳腺病灶切除和活检中的应用疗效。结果 术后病理学诊断：乳腺癌5例，乳腺纤维腺瘤77例，乳腺导管内乳头状瘤8例，乳腺增生33例，非哺乳期乳腺炎1例，乳腺癌患者接受开放性手术切除。所有病理诊断良性病变患者术后6个月接受彩色超声复查，未发现手术残留病灶。术后并发症包括皮下淤血5例，全部病例均未发生感染。结论 应用Mammotome真空辅助微创旋切系统在彩色超声引导对乳腺病灶可进行微创完整切除疗效好，并能获得精准外科治疗。

Abstract：Objective To study the Mammotome system in color ultra precise breast swollen resection of the lesions and biopsy guided by the application of curative effect. Methods The First Affiliated Hospital of Xinjiang Medical University in January 2015～June 2016， 124 cases of breast lesions in patients with 144 lesions in color ultrasound-guided Mammotome resection under， retrospective analysis and evaluate the application effect of excision and biopsy in breast lesions. Results Postoperative pathological diagnosis： 5 cases of breast cancer， 77 cases of breast fibroadenoma of breast intraductal papilloma， 8 cases of hyperplasia of mammary glands， 33 cases of lactation mastitis in 1 case， breast cancer patients undergoing open surgery. All pathological diagnosis for 6 months postoperatively in patients with benign lesions of color ultrasound review， found no surgery residual lesions. Postoperative complications included subcutaneous ecchymosis in 5 cases， has not been all cases of infection. Conclusion Mammotome vacuum assisted minimally invasive rotary system in the color of breast lesions guided by ultrasound can be a complete resection of minimally invasive good curative effect， and can obtain precise surgical treatment.

科學技术的不断进步和医学电子技术不断革新，使得新型医疗技术在医学领域相继应用。如何做到精准外科治疗，对乳腺外科治疗提出新的挑战。为减小皮肤的手术创口，避免术中出血、缩短住院时间、使患者恢复快，并且对病灶定位准确，才能真正的做到精准外科手术治疗。目前微创精准外科治疗已广泛应用于腰间盘突出、胆囊炎、前列腺增生等疾病的外科治疗中。乳腺病灶包括乳腺纤维瘤、乳腺导管内乳头状瘤、乳腺结节和乳腺多灶性病变，是女性常见的良性疾病，目前，除去传统开放手术，微创手术是治疗方法之一[1-4]。Mammotome系统是由美国强生公司生产的可在彩色超声引导下完成乳腺病灶切除及活检的系统，主要是由旋切刀和真空抽吸泵两大装置组成，通过计算机控制的真空辅助高速旋切设备，对乳腺病灶或可疑病灶可进行重复切割，以获取乳腺病灶足够的组织学标本，为乳腺疾病的病灶微创治疗和病灶诊断活检提供了更好的方法[5-7]。本研究通过对我院于2015年1月～2016年6月收治的124例乳腺病灶患者行Mammotome真空辅助微创旋切系统病灶切除，目的是探讨此项微创技术的临床应用疗效：

1 资料与方法
1.1一般资料 2015年1月～2016年6月在新疆医科大学第一附属医院接受Mammotome真空辅助微创旋切系统切除的乳腺病灶患者124例。所有患者均为女性，年龄22～71岁，中位年龄为43岁。124例患者共144个病灶，病灶数目1～3个/例；单侧乳房87例，双侧乳房37例。病灶大小0.8～3.0 cm。BI-RADS分级为II-IVa级，术前常规检查血常规及术前凝血检查且为正常范围，术前及术中非月经期。所有患者术前均行彩色超声检查。
