摘要：目的 提高病案个人信息正确率，确保病案质量与服务质量。方法 列举病案中个人信息问题。结果 姓名、性别、关系、国籍错，地址混乱，年龄不精确。分析：收录信息方法落后；对重要性认识不足，疏于核对；三级医生负责制落实不足；电子病案系统不健全。结论 只有全员重视；改进收录方法；健全三级医生负责制，加强培训；完善电脑系统；才能提高正确率、更好服务大众。
