磁共振成像和血清铁蛋白评估铁过载的对比研究(1)

     摘要:目的 本研究选取出现铁蛋白升高的血液病患者,比较肝脏T2*与血清铁蛋白评估血液病患者铁过载的特征,以确定更为合适的铁过载评估方法。方法 选取2015年11月~2017年1月就诊于安徽医科大学的血液病患者,测定患者肝脏T2*值及血清铁蛋白浓度,并对收集到的数据进行分析处理。结果 14例患者中男5例,女9例,肝T2*中位数为4.24(1.52~14.32)ms,有12例患者(85.70%)肝脏存在铁过载。14例患者的肝T2*与血清铁蛋白显著负相关(r=-0.547,P=0.043),肝T2*与输血量之间呈线性相关(r2=0.67,P=0.01),而血清铁蛋白与输血量没有相关性(r2=0.299,P=0.051)。无祛铁治疗的10例患者肝T2*与血清铁蛋白显著相关(r=-0.866,P=0.001),肝T2*、血清铁蛋白与输血量呈线性相关(r2=0.6,P=0.009;r2=0.63,P=0.006)。结论 肝脏T2*与血清铁蛋白之间存在显著的负相关性,肝脏T2*值更能准确的描述铁过载情况,对轻度铁过载更敏感,可信度及重复性更佳。

    Abstract:Objective In this study,patients with elevated serum levels of ferritin were selected to compare the characteristics of iron overload in the liver with T2* and serum ferritin to determine the more appropriate iron overload assessment method.Methods From November 2015~January 2017 in medical university of Anhui hospital blood disease patients were measured T2* value of liver and serum ferritin concentrations,and the collected data were analyzed.Results In 14 cases,male 5 cases,female 9 cases,liver T2* was 4.24(1.52~14.32) ms,12 patients(85.7%)liver iron overload.14 patients with liver T2* and serum ferritin were significantly negative correlation(r=-0.547,P=0.043),there was a linear correlation between the liver T2* and blood transfusion(r2=0.67,P=0.01),liver T2* and serum ferritin and blood transfusion had no correlation(r2=0.299,P=0.051).No significant related to iron chelation therapy in 10 patients liver T2* and serum ferritin(r=-0.866,P=0.001),liver T2*,a linear correlation between serum ferritin and blood transfusion volume(r2=0.6,P=0.009;r2=0.63,P=0.006).Conclusion There was a significantly negative correlation between the conclusion of the liver T2* and serum ferritin,liver T2* could more accurately describe the iron overload situation,more sensitive to mild iron overload,reliability and better repeatability.

    骨髓增生异常综合征、再生障碍性贫血等血液系统疾病,其患者往往因骨髓造血功能异常,而需要长期输血治疗。尽管这些治疗有着救命的好处,但输血可以引起进一步的并发症,特别是铁过载。铁过载危害巨大,产生氧自由基攻击靶器官的细胞膜、DNA,最终引起靶器官功能障碍,临床上表现为心力衰竭、肝硬化等,严重影响患者的生活质量和生命安全[1]。对于铁过载的患者来说,需要不断检测体内铁负荷并及时行祛铁治疗。磁共振成像是一种非侵袭性技术,已经被认为是一个可靠的方法检测不同器官如肝与心铁沉积情况。有研究证实了使用MRI测量肝铁含量和肝脏活组织检查检测量肝铁含量有良好的相关性[2]。, 百拇医药(李陈芃 翟志敏 陶千山)
