LEEP刀与冷刀锥切治疗宫颈上皮内瘤变的效果分析(1)
摘 要：目的 探讨子宫颈上皮内瘤变Ⅲ级的患者采用环形高频电波刀与冷刀锥切治疗的临床效果。方法 选取我院CINⅢ级伴高危型人乳头瘤病毒感染患者98例，按患者门诊号单双数分为两组，单号為观察组，双号为对照组，每组49例。观察组采用LEEP刀治疗，对照组采用CKC治疗。观察两组患者手术情况，采用日常生活评分评估两组患者术后生活质量情况。随访1年，观察患者预后情况和HPV阴转情况。结果 观察组患者手术时间、出血量、创面愈合时间上均优于对照组(P<0.05)。两组患者治疗前生活质量评分，差异无统计学意义(P>0.05)；治疗后观察组 ADL 评分高于对照组(P<0.05)。随访患者1年，观察组与对照组好转率，差异无统计学意义(P>0.05)；而观察组并发症发生率为4.08%，低于对照组16.33%，差异有统计学意义(P<0.05)。观察组LEEP治疗后6个月HPV阳性率16.33%，低于观察组22.45%，差异有统计学意义(P<0.05)。结论 LEEP 治疗CINⅢ效果与CKC相似，但手术操作更简便，创伤更小，术后不良反应更小，恢复更佳，值得临床应用及推广。

Abstract：Objective To investigate the clinical effect of loop electrosurgical knife and cold knife conization for patients with grade III cervical intraepithelial neoplasia.Methods 98 patients with high risk human papillomavirus infection(CIN Ⅲ)in our hospital were divided into two groups according to the number and even number of outpatient service.The single number was the observation group and the double number was the control group(49 cases in each group).The observation group was treated with LEEP knife and the control group with CKC.The operation status of the two groups was observed，and the life quality of the two groups was evaluated by the daily life score.Follow up for 1 years，the prognosis and HPV negative conversion were observed.Results The operative time， bleeding volume and wound healing time of the observation group were better than that of the control group (P<0.05).There was no significant difference in the quality of life between the two groups before treatment(P>0.05).After treatment，the ADL score of the observation group was higher than that of the control group(P<0.05).After 1 years of follow-up，there was no significant difference in the improvement rate between the observation group and the control group(P>0.05)，while the incidence of complications in the observation group was 4.08%，lower than that of the control group 16.33%，with a statistically significant difference(P<0.05).In the observation group，the positive rate of HPV was 16.33% in the 6 months after LEEP treatment，which was lower than that in the observation group 22.45%，and the difference was statistically significant(P<0.05).Conclusion The effect of LEEP for the treatment of CIN III is similar to that of CKC，but the operation is more convenient，less traumatic，less adverse reaction after operation，better recovery，and worthy of clinical application and popularization., 百拇医药(钟书平)
