不同证型变应性鼻炎外周血清中维生素D、IL—4及IFN—γ水平比较(1)
摘 要：目的 探讨血清1，25羟维生素D3、血清IL-4、IFN-γ水平与不同中医证型变应性鼻炎(AR)的相关性。方法 应用高效液相色谱串联质谱仪测定4组不同中医证型变应性鼻炎患者各30例及30例正常体检患者血清1，25羟维生素D3，ELISA酶联免疫法测定各组血清IL-4、IFN-γ水平。结果 4种不同中医证型的变应性鼻炎组血清1，25羟维生素D3、血清IFN-γ水平低于健康对照组，而血清IL-4水平则高于健康对照组，差异具有统计学意义(P<0.05)；不同证型的变应性鼻炎患者之间三种因子水平比较，差异无统计学意义(P>0.05)。结论 血清1，25羟维生素D3缺乏或不足与变应性鼻炎具有密切的关系，其可能作用机制为血清1，25羟维生素D3缺乏可导致機体Th1/Th2细胞因子分泌失衡，刺激IgE水平升高引起变态反应。不同证型的变应性鼻炎患者血清1，25羟维生素D3、血清IL-4、IFN-γ水平大致相当。

Abstract：Objective To investigate the correlation between serum 1，25-hydroxyvitamin D3，serum IL-4 and IFN-γ levels and different TCM syndromes of allergic rhinitis(AR).Methods The levels of 1，25-hydroxyvitamin D3 in serum of 30 patients and 30 normal subjects in 4 groups of TCM syndrome allergic rhinitis were measured by high performance liquid chromatography tandem mass spectrometry.The levels of serum IL-4 and IFN-γ in each group were determined by ELISA enzyme-linked immunosorbent assay.Results The levels of serum 1，25 hydroxyvitamin D3 and serum IFN-γ in allergic rhinitis group were lower than those in healthy control group，and serum IL-4 levels were higher than those in healthy control group，the difference was statistically significant(P<0.05)；There was no significant difference in the levels of the three factors between patients with allergic rhinitis of different syndrome types (P>0.05).Conclusion The lack or deficiency of serum 1，25 hydroxyvitamin D3 is closely related to allergic rhinitis， and its possible mechanism is that the deficiency of serum 1，25 hydroxyvitamin D3 may lead to the imbalance of Th1/Th2 cytokine secretion in the body and stimulate the allergic reaction by increasing the level of IgE.The levels of serum 1，25 hydroxyvitamin D3， serum IL-4 and IFN- gamma in patients with allergic rhinitis were similar.

变应性鼻炎(allergic rhinitis，AR)是耳鼻咽喉科常见病及多发病，且其发病机制还未完全明确，以突然和反复发作的鼻痒、打喷嚏、流清涕、鼻塞为主要临床症状。中医学认为变应性鼻炎属于“鼻鼽”范畴。临床上多数学者认为本病可分为肺气虚寒、脾气虚弱、肾阳不足和肺经伏热四种证型。近年研究指出，变应性鼻炎与Th1/Th2功能失衡关系密切[1]。维生素D已不再是单纯的维生素，且被证实可以通过参与调节免疫通路，进而治疗多种自身免疫疾病，因此维生素D是否与变应性鼻炎亦存在关联性，目前国内对维生素D水平与变应性鼻炎关系的临床研究甚少，且结论不一，因此该问题值得研究探讨。为此，在吸取前人研究成果的基础上，本课题拟从观察不同证型变应性鼻炎患者血清维生素D、IL-4及IFN-γ水平是否有差异及其相互关系，为变应性鼻炎的临床诊治提供试验依据，间接促进中医药防治变应性鼻炎的临床推广应用。
1资料与方法
1.1一般资料 选择福建中医药大学附属第二人民医院耳鼻咽喉科门诊2017年1月～12月就诊的不同证型常年性变应性鼻炎患者各30例，作为试验组，共120例，其中男64例，女56例，年龄19～58岁，平均年龄(27.12±1.30)岁。选择我院体检中心健康体检者30例为健康对照组，其中男17例，女13例，年龄20～61岁，平均年龄(28.10±0.81)岁，所有入组者的变应性鼻炎的症状、体征得分均为0分。两组患者的年龄、性别等一般临床资料比较，差异无统计学意义(P>0.05)，具有可比性。, 百拇医药(许亚桦)
