血清IL—6、IL—10、CRP和PCT水平检测对手足口病早期合并细菌感染患儿的临床诊断价值(1)
摘 要：目的 探讨血清IL-6、IL-10、CRP和PCT水平检测对手足口病早期合并细菌感染患儿的临床诊断价值。方法 选取我院收治的早期手足口病患儿90例为观察组，并根据患儿是否合并细菌感染分为单纯组51例和感染组39例，选取同期来我院体检的健康儿童50例为对照组，比较三组间IL-6、IL-10、CRP和PCT水平差异。结果 观察组患儿IL-6、IL-10、CRP以及PCT水平高于对照组，差异有统计学意义(P<0.05)；感染组患儿CRP以及PCT水平高于单纯组，差异具有统计学意义(P<0.05)；感染组和单纯组IL-6、IL-10水平比较，差异无统计学意义(P>0.05)。结论 IL-6、IL-10水平在鉴别手足口病是否合并细菌感染方面的意义有待于进一步研究，而联合CRP和PCT检测可为临床鉴别早期手足口患儿是否合并细菌感染提供可靠的实验室依据，利于临床早期用药治疗。

Abstract：Objective To investigate the clinical value of serum IL-6，IL-10，CRP and PCT in the diagnosis of hand-foot-and-mouth disease(HFMD)complicated with bacterial infection.Methods 90 children with early hand-foot-and-mouth disease admitted to our hospital were selected as observation group，and according to whether the children had bacterial infection，51 cases were simple group and 39 cases were infected group.50 healthy children who were admitted to our hospital during the same period were selected as the control group，and the differences of IL-6，IL-10，CRP and PCT levels were compared between the three groups.Results The levels of IL-6，IL-10，CRP and PCT in the observation group were higher than those in the control group，the difference was statistically significant(P<0.05).The levels of CRP and PCT in the infected group were higher than those in the simple group，the difference was statistically significant(P<0.05).There was no significant difference in the levels of IL-6 and IL-10 between the infected group and the simple group(P>0.05).Conclusion The significance of IL-6 and IL-10 levels in the identification of hand-foot-and-mouth disease with bacterial infection remains to be further studied，and combined CRP and PCT testing can provide a reliable laboratory basis for clinical identification of early hand-foot-and-mouth disease children with bacterial infection for the early clinical treatment.

手足口病(hand-foot-and-mouth disease，HFMD)屬于急性传染性疾病，多发于3岁以下儿童，约70%患儿是由柯萨奇病毒和肠道病毒引起。临床症状主要表现为发热、手足臀等部位及口腔出现斑丘疹或疱疹。其中大部分患儿症状较轻，可自行愈合，但少数病情严重者可继发细菌感染，引起肺水肿、心肌炎、无菌性脑膜脑炎等并发症，严重影响患儿生命健康[1]。早期对患儿的病情做出判断，提前给予干预措施，可有效控制病情发展，改善预后。本研究选取我院收治的手足口病患儿为研究对象，探讨血清白介素-6(IL-6)、白介素-10(IL-10)、C反应蛋白(CRP)和降钙素原(PCT)水平检测的临床应用价值，现将结果总结报告如下。, 百拇医药(管建 姜娟娟)
