血清NSE及UCB检测新生儿高胆红素血症脑损伤的价值(1)

     摘 要:目的 探討新生儿高胆红素血症脑损伤临床诊断中血清神经元特异性烯醇化酶及脑脊液未结合胆红素联合颅脑MRI的使用价值。方法 选取2017年1月~12月我院收治的神经系统损伤高胆红素血症患儿36例为实验组,另选同期住院的无神经系统损伤的高胆红素血症患儿36例为对照组,对两组患儿进行血清NSE及UCB指标进行检测并进行对比观察,对实验组患儿进行MRI检查,并根据检查结果分为MRI正常组及异常组,对比观察两组患儿血清学指标变化情况,同时对两种检测方法进行相关性分析。结果 实验组患儿血清NES及UCB水平高于对照组患儿,差异具有统计学意义(P<0.05)。经颅脑MRI检查发现,实验组36例患儿中28例存在异常,MRI异常患儿血清NES指标高于无异常患儿,差异具有统计学意义(P<0.05)。对MRI检测结果与血清学指标进行相关性分析发现,血清NES水平与MRI检出率存在正相关关联(r=0.397,P<0.05)。结论 血清NSE、UCB及MRI检查是高胆红素伴脑损伤患儿检测的重要标志,可进行早期诊断并对患儿预后进行评估。

    The Value of Serum NSE and UCB in Detecting Brain Injury in Neonates with Hyperbilirubinemia

    Abstract:Objective To investigate the value of serum neuron specific enolase and unbound bilirubin in cerebrospinal fluid in the clinical diagnosis of neonatal hyperbilirubinemia.Methods From January to December 2017,36 children with hyperbilirubinemia with nervous system injury were selected as experimental group and 36 children with hyperbilirubinemia without nervous system injury as control group.The serum NSE and UCB were detected and compared between the two groups,and the MRI examination was performed on the children in the experimental group.According to the results,they were divided into two groups:MRI normal group and abnormal group.The changes of serological indexes in the two groups were compared and observed.At the same time,the correlation between the two detection methods was analyzed.Results The levels of serum NES and UCB in the experimental group were significantly higher than those in the control group,the difference is statistically significant(P<0.05).MRI examination of the brain showed that there were abnormalities in 28 of the 36 patients in the experimental group.The serum NES index of the children with MRI abnormalities was higher than that of the children without abnormalities,the difference was statistically significant(P<0.05).Correlation analysis between MRI test results and serological indicators revealed a positive correlation between serum NES levels and MRI detection rates(r=0.397, P<0.05).Conclusion Serum NSE,UCB and MRI are important markers for the detection of hyperbilirubinemia with brain injury in children.They can be used for early diagnosis and prognosis evaluation., 百拇医药(邬剑 李慧玲)
