胫腓骨严重开放粉碎性骨折患者采用外固定支架固定结合VSD负压吸引技术治疗的效果分析(1)
摘要：目的 探討胫腓骨严重开放粉碎性骨折患者采取外固定支架固定结合VSD负压吸引术治疗的价值。方法 选取我院于2016年7月～2018年2月收治的78例非胫骨严重开放粉碎性骨折患者，将其随机分为对照组与观察组，各39例。其中对照组予以单纯外固定支架治疗术，观察组则予以外固定支架固定联合VSD负压吸引治疗术。对比两组患者的骨折恢复优良率，手术指标以及并发症的发生情况。结果 观察组骨折恢复优良率(92.31%)高于对照组(66.67%)，差异有统计学意义(P<0.05);观察组术后并发症发生率(7.69%)低于对照组(33.33%)，差异有统计学意义(P<0.05);观察组术中出血量(103.21±24.63)ml少于对照组(184.56±30.46)ml，住院时间(27.84±5.66)d短于对照组(37.82±6.38)d，手术时间(71.05±7.89)min则短于对照组(92.14±8.32)min，差异有统计学意义(P<0.05)。结论 针对胫腓骨严重开放粉碎性骨折患者的治疗过程中，给予患者外固定支架联合VSD负压吸引治疗术的治疗方式，不仅能显著提高骨折恢复优良率，降低术后并发症的发生率，值得临床借鉴实施。

Methods 78 patients with severe open comminuted fractures of the humerus were enrolled in our hospital from July 2016 to February 2018. They were randomly divided into control group and observation group， 39 cases each. The control group received simple external fixation and the observation group was treated with external fixation and VSD vacuum aspiration. The excellent rate of fracture recovery， surgical indexes and complications were compared between the two groups. Results The excellent rate of fracture recovery in the observation group (92.31%) was higher than that in the control group (66.67%)，the difference was statistically significant (P<0.05). The postoperative complication rate (7.69%) was lower in the observation group than in the control group (33.33%)， the difference was statistically significant (P<0.05); the intraoperative blood loss (103.21±24.63) ml in the observation group was lower than that in the control group (184.56±30.46) ml， and the hospitalization time (27.84±5.66)d was longer than the control group (37.82±6.38)d，the operation time (71.05±7.89) min was shorter than that of the control group (92.14±8.32) min，the difference was statistically significant (P<0.05). Conclusion In the treatment of patients with severe open comminuted fracture of the humerus， the treatment of external fixation combined with VSD vacuum aspiration therapy can not only significantly improve the excellent rate of fracture recovery， but also reduce the incidence of postoperative complications.
