艾滋病合并淋巴瘤的临床病例分析(1)

     摘要:目的 探究艾滋病合并淋巴瘤的病例临床特征。方法 选取2014年2月~2019年2月的25例艾滋病合并淋巴瘤患者为研究对象,分析患者临床表现、免疫指标、临床病理,患者的治疗效果及治疗前后生活质量评分。结果 25例患者均有浅表淋巴结肿大,23例间断性发热,20例体重下降。其中18例患者因肿瘤就诊,经HIV筛查确诊艾滋病,6例为先感染艾滋病,后发现肿瘤;CD4+T淋巴细胞为21~391 g/μl,平均为(136.74±21.78)g/μl;临床分期为Ⅰ期8例(32.00%),Ⅱ期7例(28.00%),Ⅲ期4例(16.00%),Ⅳ期6例(24.00%);治疗有效率为15例(60.00%)。治疗后患者的生活质量评分为(17.45±2.72)分,低于治疗前的(24.78±2.67)分,差异有统计学意义(P=0.0000)。结论 艾滋病合并淋巴瘤伴随多种临床症状,其免疫力低下,需及时给予有效治疗,改善预后。

    Abstract:objective To investigate the clinical features of cases of AIDS with lymphoma. Methods 25 patients with AIDS and lymphoma from February 2014 to February 2019 were enrolled in the study. The clinical manifestations, immune parameters, clinical pathology, treatment effect and quality of life before and after treatment were analyzed. Results 25 patients had superficial lymphadenopathy, 23 patients had intermittent fever, and 20 patients had weight loss. Among them, 18 patients were diagnosed with cancer due to tumor diagnosis, 6 cases were diagnosed with AIDS by HIV screening, and 6 cases were infected with AIDS. CD4+ T lymphocytes were 21~391 g/μl, with an average of (136.74±21.78) g/μl; The clinical stage was 8 cases (32.00%) in stage I, 7 cases (28.00%) in stage II, 4 cases (16.00%) in stage III, and 6 cases (24.00%) in stage IV. The effective rate was 15 cases (60.00%). The quality of life score of the patients after treatment was (17.45±2.72) points, which was lower than that before treatment (24.78±2.67), and the difference was statistically significant (P=0.0000). Conclusion AIDS complicated with lymphoma is associated with a variety of clinical symptoms, and its immunity is low. It is necessary to give effective treatment in time to improve the prognosis.

    艾滋病(AIDS)是一种常见性病,其传播途径为血液传播、性行为传播、母婴传播等,致死率较高,严重威胁生命安全[1]。随着人们思想观念改變,性病发生率呈上升趋势,AIDS男性发病率较高,受到关注。由于患者免疫缺陷严重,极易引发多种机会性感染,发生相关肿瘤,如宫颈癌、卡波西肉瘤、淋巴瘤等[2]。目前,关于AIDS合并淋巴瘤的研究较少,本研究将以我院收治的25例AIDS合并淋巴瘤患者为对象,探究该病的临床特征,现将结果报道如下。

    1资料与方法

    1.1一般资料 选取2014年2月~2019年2月深圳市第三人民医院收治的25例AIDS合并淋巴瘤患者为研究对象,其中男20例,女5例;年龄34~51岁,平均年龄(42.72±4.64)岁。本研究经医院伦理委员会审批,患者均熟知本次研究,并签署知情同意书。纳入标准:①资料齐全;②年龄超过18岁。排除标准:①其他恶性肿瘤、免疫系统疾病、心肺等重要器官功能障碍;②不配合研究者。, http://www.100md.com(曹廷智)
