【摘要】目的：探讨从医院到社区无缝隙护理管理模式在慢性心力衰竭(CHF)患者治疗中的应用。方法：对124例CHF患者分组护理，对比护理效果。结果：观察组生活质量评分与心功能各指标均明显较对照组理想(P<0.05)。结论：CHF患者应用从医院到社区无缝隙护理管理模式效果良好。

    [Abstract] Objective： Discussion on treatment of patients from hospitals to community care seamless management of chronic heart failure (CHF) in applications..Methods： 124 patients with CHF group care， nursing contrast effect. Results： Observation group quality of life score and cardiac function indexes were significantly higher than the control group over (P <0.05). Conclusion： CHF patients from hospitals to community care seamless management to good effect.
