    【摘要】目的：探究白細胞计数联合外周血镜检、降钙素原、纤维蛋白原检测诊断老年患者细菌感染的敏感性及特异性。方法：研究对象选取我院2015年1月至2018年5月收治的80例疑似细菌感染老年患者，使用随机数字表法分为三联组(白细胞计数WBC、降钙素原PCT、细菌培养)40例，及四联组(白细胞计数WBC联合外周血镜检、降钙素原PCT、纤维蛋白原FIB)40例，另取20例同期健康老年人作为健康对照组，比较健康对照组与细菌感染老年患者白细胞计数(WBC)、降钙素原(PCT)及纤维蛋白原(FIB)水平。比较三联组与四联组检测阳性、阴性预测值，特异度、灵敏度及约登指数。结果：老年患者细菌感染WBC、PCT、FIB水平均显著高于健康对照组(P<0.05)；四联组检测阳性预测值、阴性预测值、灵敏度、特异度、约登指数均显著高于三联组，差异具有统计学意义(P<0.05)。结论：老年患者细菌感染可引起其WBC、PCT、FIB水平上升，使用WBC、外周血镜检、PCT、FIB联合检测能有效提高老年细菌感染患者诊断阳性、阴性预测值及诊断灵敏度、特异度、约登指数，值得临床推广。
