加速康复外科在手背深度烧伤植皮手术治疗中的应用研究(1)
[摘要]目的：探讨加速康复外科在手背深度烧伤植皮手术治疗中的应用及其对患者预后的影响。方法：选取2016年11月-2017年11月笔者医院收治的手背深度烧伤植皮患者138例，随机分为两组，对照组应用常规康复外科，研究组应用加速康复外科。比较两组患者平均手指关节活动度、皮片下血肿及手术植皮成活情况、手功能恢复情况、疼痛程度、生活质量。结果：研究组平均手指关节活动度、皮片下血肿及手术植皮成活情况均优于对照组，差异有统计学意义(P<0.05)。研究组手功能恢复各项指标及生活质量各项评分均优于对照组，差异有统计学意义(P<0.05)。研究组术后各时间点VAS评分均小于对照组，差异有统计学意义(P<0.05)。结论：在手背深度烧伤植皮手术治疗的过程中，应用加速康复外科治疗，效果较好，值得在临床上不断推广应用。

Abstract： Objective To explore the application of accelerated rehabilitation surgery for skin grafting of deep burn on the back of hand and its effect on the prognosis of patients. Methods 138 cases of skin graft of deep burn on the back of hand treated in our hospital from November 2016 to November 2017 were selected and randomly divided into two groups， the control group applied conventional rehabilitation surgery and the research group applied accelerated rehabilitation surgery. The average finger joint activity， subcutaneous hematoma， skin graft survival， hand function recovery， pain degree and life quality were compared between the two groups. Results The mean finger joint mobility， subcutaneous hematoma and skin grafting survival of the study group were better than those of the control group， and the difference was statistically significant(P<0.05). The scores of hand function recovery and quality of life in the study group were better than those in the control group(P<0.05). The VAS scores of the study group were lower than those of the control group at all time points(P<0.05). Conclusion In the process of skin grafting for deep burn on the back of hand， accelerated rehabilitation surgery is effective， which should be popularized and applied in clinic.

手背烧伤是临床工作中较常见的外伤，其发生率一直呈逐渐增长趋势。手背是人体中较薄的皮肤组织，面积较大，通常创面都较为严重，临床上又将其称为深度烧伤[1]。手背烧伤后如果愈合不完全，会遗留较多瘢痕，严重影响患者的整体形象，对患者的外观造成一定影响。另外，在手背深度烧伤患者的治疗中，如没有进行有效、科学的治疗手段，会对患者后期的手功能恢复产生一定的影响，降低预后，严重降低患者的生活质量[2]。手背深度烧伤面积较大，无法完全愈合，如果及时植皮修复，可有效改善愈合程度。在治療过程中，一定要根据患者的实际情况选择行之有效的治疗措施，并且进行一定的康复措施对于提高治疗效果[3]。笔者医院在手背深度烧伤植皮手术治疗过程中，应用加速康复外科，效果理想，现报道如下。

1 资料和方法
1.1 一般资料：选取2016年11月-2017年11月笔者医院收治的手背深度烧伤植皮患者138例，随机分为两组，每组69例。纳入标准：①经诊断均符合手背深度烧伤手术植皮的诊断标准；②手背深度烧伤患者；③均知晓同意此次研究。排除标准：①配合度低者；②合并有其他重大疾病者；③精神失常者；④手背轻度烧伤患者。对照组：男36例，女33例；年龄24～50岁，平均(35.5±1.4)岁；烧伤面积为(70.5±16.2)% TBSA；深Ⅱ度24例，深Ⅲ度45例。研究组：男40例，女29例；年龄23～52岁，平均(36.1±1.1)岁；烧伤面积为(69.8±17.9)% TBSA；深Ⅱ度30例，深Ⅲ度39例。两组患者一般资料比较，差异有统计学意义(P>0.05)，具有可比性。此次研究经过医院伦理委员会批准。, 百拇医药(李霜 张红娇)
