薄角膜瓣Lasik分区切削治疗高度近视的临床探讨(1)

     【摘要】目的:评价薄角膜瓣下应用分区切削模式进行lasik治疗高度近视的临床效果。方法:对118例(220眼)高度近视患者因选择常规切削剩余角膜厚度小于250um,而进行薄瓣下分区切削治疗,分2~3区切削,切削光区4.5~6.0mm,随访观察6月~48月,观察术前后的裸眼视力,矫正视力,屈光状态及并发症的发生情况。结果:薄瓣Lasik分区切削术后反应轻,裸眼视力恢复快,术后第一天裸眼视力即明显提高,术后6月裸眼视力在1.0以上者有197眼占89.5%,0.5~0.8者22眼占10%,小于0.5者1眼占0.5%,屈光度稳定在±1.00D以下者191眼占86.8%。主要并发症为角膜瓣细微皱褶,少数患者出现眩光。夜视力下降,一月后减轻或消失。结论:薄瓣准分子激光原位角膜磨镶术分区切削模式治疗角膜相对较薄的高度近视患者,可节省角膜组织,具有安全可靠,稳定性﹑可预测性强的临床效果。 

    Clinical Investigation of multi-zone ablation in thin cornea flap Lasik for high myopia 

    【Abstract】Objective: To evaluate the clinical effects of applying multi-zone ablation mode Lasik treating high myopia in the thin cornea flap. Methods: 118 patients (220 eyes) with high myopia had treatment of multi-zone ablation in the thin cornea flap due to the remaining corneal thickness were less than 250um selecting the conventional ablation. The multi-zone ablation divided 2 or 3 sub-area and the ablation Light District were 4.5 ~ 6.0mm.The uncorrected visual acuity before and after operation, corrected visual acuity, refractive status and the incidence of complications were observed following 6 to 48 months. Results: Thin flap Lasik provides less discomfort and faster visual recovery. The uncorrected visual acuity improved evidently on the first day after operation. for 6 months, there were 197 eyes with visual acuity 1.0 or more accounted for 89.5%, 22 eyes with visual acuity 0.5~0.8 accounted for 10% and only 1 eye with visual acuity less than 0.5 accounted for 0.5%. There were 191 eyes which refraction stabilized below ±1.00D, accounted for 86.8%. The minor cornea flap folds were the main complication, a small number of patients glared. The night vision decreased, but it reduced or disappeared after 1 month. Conclusion: Thin flap laser in-situ keratomileusis multi-zone ablation treated patients with high myopia relatively thin cornea. It can save the cornea organizations securely, and had reliable, stable, predictable strongly clinical effects.
