【摘要】目的：探讨全髋关节置换翻修术后康复护理的效果。方法：对2006年2月至2011年4月行各类人工全髋关节置换翻修术后的61例患者随机分为两组,第1组(康复组)给予患者的特殊康复治疗,第2组(普通组)术后仅给予牵引、常规的术后护理。于术后1个月、3个月及6个月时分别用Harris髋关节功能评分对两组患者进行功能评定；观察患者术后的髋关节发生脱位和深静脉血栓的情况。结果：两组患者术后1个月时髋关节的功能评分没有显著性差异(P0.0878，但术后3个月及6个月时康复组功能评分明显高于非康复组(P<0.0001。康复组1例髋关节脱位，非康复组没有发生髋关节脱位；康复组没有深静脉血栓的发生，非康复组有6例出现深静脉血栓。结论：对人工全髋关节置换翻修患者给予遵循个体化、渐进性、全面性的康复护理有利于提高患者术后的髋关节功能和减少深静脉血栓的发生率。

Rehabilitation nursing for patients underwent revision of the totally replaced hip

【Abstract】Objective：To investigate the effects of rehabilitation nursing after revision of the totally replaced hip(RTHR). Methods：Sixty-one patients with various revision operations on the totally replaced hip were randomly divided into 2 groups.The patients of 1st group were give rehabilitation nursing and the patients of 2nd group were give conventional nursing and traction after operation. Harris score was used to evaluate the function of hip in two groups respectively 1 month,3 months and 6 months after operation. Furthermore, the deep vein thrombosis and hip dislocation occurred in the patients were also investigated. Results：The Harris score of two groups had no significant difference by the end of 1 month after operation (P0.0878).But the Harris score of 1st group(rehabilitation nursing group) was obviously higher than group 2nd group(conventional nursing group) by the end of 3 months and six month after operation ( P < 0. 0001).There was 6 cases suffered deep vein thrombosis in conventional nursing group and 1 case suffered hip dislocation in rehabilitation nursing group after operation. Conclusion：The individual, progressive and general rehabilitation nursing with a focus on the muscle strengthening， especially of the abductor muscle will benefit for the improving of hip function and reducing the occurring of deep vein thrombosis of the patients who had revision of totally replaced.

全髋关节置换术后的康复护理提高患者术后使髋关节功能恢复外许多学者的一致肯定〖1-3〗。但对全髋关节置换翻修术而言,由于患者病情重、复杂，手术特殊，术后的康复护理方面研究较少,对此我们对其进行了随机对比研究，现报告如下：
1 资料与方法
1.1 临床资料:2006年2月至2011年4月在我院骨科施行RTHR的患者61 例,其中男20例，女41 例；年龄为41～72岁，平均62.47岁。翻修原因为髋臼和股骨假体无菌性松动47例(其中单纯髋臼假体松动8例，单纯股骨假体松动26例，髋臼和股骨假体同时松动13例)，髋臼聚乙烯内衬脱位3例 ......
