2型糖尿病合并肺结核的临床特点及治疗(1)

     [摘要] 目的 探讨2型糖尿病合并肺结核的临床特点及治疗手段。方法 选取该院2014年1月—2015年2月所收治的2型糖尿病合并肺结核患者82例作为研究的对象,对患者的临床症状、痰菌涂阳率等进行观察,了解其临床特点,同时按随机数字表法均分成实验组与对照组,前者给予血糖控制+抗结核治疗,后者仅实施抗结核治疗,对比两组患者的治疗效果。结果 患者临床表现咳嗽74例,咳痰58例,发热39例,盗汗21例,咯血38例,呼吸困难25例,空洞43例;痰菌检查36例呈现阳性;影像学检查一个肺野57例,两个肺野17例,三个肺野及其以上8例。实验组患者治疗后的总有效率(85.4%)高于对照组(61.0%),差异有统计学意义(P<0.05)。 结论 2型糖尿病合并肺结核的临床特点主要表现为咳嗽、咳痰、发热、盗汗、痰菌检查为阳性等;对此类患者给予抗结核+血糖控制治疗,可以显著提高治疗效果,值得在临床上大力推行与运用。

    Clinical Features and Treatment of Type 2 Diabetes Complicated with Pulmonary Tuberculosis

    [Abstract] Objective To explore the clinical features and treatment of type 2 diabetes complicated with pulmonary tuberculosis. Methods 82 patients with type 2 diabetes complicated with pulmonary tuberculosis admitted in our hospital from January 2014 to February 2015 were selected as the subjects. The clinical symptoms and sputum smear positive rate were observed in both groups so as to understand the clinical features. The patients were equally divided into the experimental group and the control group. Patients in the experimental group were given the glycemic control and antituberculous treatment, while those in the control group were only given the antituberculous treatment. And the results of therapy of the two groups were compared. Results Of all the patients, there were 74 cases with cough, 58 cases with expectoration, 39 cases with fever, 21 cases with night sweat, 38 cases with hemoptysis, 25 cases with dyspnea and 43 cases with cavity. The result of sputum examination showed that there were 36 positive cases. The result of imaging examination showed 57 cases with 1 diseased lung field, 17 cases with 2 diseased lung fields, and 8 cases with 3 or more diseased lung fields. The overall effective rate was much higher in the experimental group than that in the control group (85.4% vs 61.0%) with was a statistically significant difference between them (P<0.05). Conclusion The clinical features of type 2 diabetes complicated with pulmonary tuberculosis are cough, expectoration, fever, night sweat and the positive result of sputum examination and so on. Antituberculous and glycemic control treatment can significantly improve the treatment effect on the disease, which is worth promoting and applying vigorously in clinical practice., 百拇医药(宋英)
