糖尿病早期肾损伤应用尿糖联合尿微量白蛋白检验诊断效果观察(1)

     [摘要] 目的 探究糖尿病早期肾损伤应用尿糖联合尿微量白蛋白(mAlb)检验诊断效果。 方法 选取2016年2月—2017年2月收治的54例糖尿病患者作为研究组,取同期健康体检正常者49名作为对照组,所有研究对象均行尿糖、尿微量白蛋白检测,比较两组联合诊断阳性率、尿糖与mAlb水平、诊断效能。结果 研究组尿糖检出阳性率为81.48%,mAlb阳性率74.07%,联合诊断阳性率为90.74%,显著高于对照组0.00%,且联合诊断较单独检验阳性率显著高,差异有统计学意义(P<0.05);研究组尿糖、mAlb与对照组相比均较高,比较差异有统计学意义(P<0.05);联合诊断特异度为66.66%(2/3),灵敏度为94.11%(48/51),准确度为92.59%(50/54)。结论 糖尿病早期肾损伤采取尿糖、mAlb联合检验有助于诊断准确率提高,从而指导临床予以早期治疗,促进患者疗效提升,具一定临床应用与研究价值。

    Observation on the Effect of Urinary Glucose Combined with Urinary Microalbumin Test in Early Diabetic Nephropathy

    [Abstract] Objective This paper tries to investigate the diagnostic effect of urinary glucose combined with urinary microalbumin (mAlb) test in early diabetic nephropathy. Methods 54 patients with diabetes mellitus from February 2016 to February 2017 were selected as the research group. 49 healthy persons were selected as the control group. All subjects were tested by urinary glucose and urinary microalbumin. Diagnosis of positive rate, urine glucose and mAlb levels, diagnostic efficacy were compared. Results The positive rate of urine test in the research group was 81.48%, the positive rate of mAlb was 74.07%, the positive rate of combined diagnosis was 90.74%, which was significantly higher than that of the control group of 0.00%, and the combined diagnosis was significantly higher than the single test(P<0.05), urine glucose and mAlb levels in the research group were significantly higher than those in the control group(P<0.05). The difference between the two groups was statistically significant (P<0.05), combined diagnosis specificity rate was 66.66% (2/3), sensitivity rate was 94.11% (48/51) and accuracy rate was 92.59% (50/54). Conclusion The combined detection of mAlb and urinary glucose in early diabetic nephropathy can help to improve the accuracy of diagnosis, so as to guide the early clinical treatment and promote the curative effect. It has certain clinical application and research value.

    糖尿病(DM)为一种自身免疫性疾病,患者机体代谢处在紊乱状态,且呈现出高血糖表现,导致患者器官损伤、功能障碍,特别是对于肾功能的损伤较大。DM患者早期肾损伤无典型性症状,易遭到忽视,随病情进展将导致血管疾病,加重病情,从而影响治疗效果与患者预后[1]。因此,临床需进行有效早期诊断,从而及早制定个性化治疗方案予以对症治疗,改善患者肾损伤程度。目前,临床予以尿糖及尿微量白蛋白(mAlb)二者联合检测,为明确其诊断效果,该院针对性选取2016年2月—2017年2月收治的54例DM患者与49名健康者臨床资料分析,现报道如下。, http://www.100md.com(王翠萍 张爱芳)
