    [摘要] 目的 探讨糖化血红蛋白(HbA1c)、空腹血糖(FPG)、尿微量白蛋白(MALB)检测对糖尿病肾病患者的临床意义。方法 选择2018年7月—2020年1月该院确诊为糖尿病的患者106例为研究组根据其是否患糖尿病肾病将其分为肾病组(n=53)和非肾病组(n=53)。对照组纳入同一时间段于该院体检的健康者为50名。对两组研究参与者均进行HbA1c、FPG、mALB的检测,就检测结果进行组间及组内的对比分析。 结果 研究组患者的HbA1c、FPG、MALB水平均高于对照组,且差异有统计学意义(P<0.05);肾病组患者的HbA1c、FPG、MALB水平均高于非肾病组(P<0.05),可知其水平与疾病严重程度正相关;3项联合检测、HbA1c与FPG联合检测、MALB单一检测对糖尿病肾病的阳性率分别为83.02%、66.04%、24.53%,3项联合检测的阳性率最高(P<0.05)。结论 HbA1c、FPG、MALB水平对糖尿病肾病患者的诊断及疾病严重程度判斷有重要临床意义,联合检测可显著提高诊断准确率。

    Significance Analysis of Glycosylated Hemoglobin, Fasting Blood Glucose and Urine Microalbumin in Patients with Diabetic Nephropathy

    [Abstract] Objective To explore the clinical significance of glycosylated hemoglobin (HbA1c), fasting blood glucose (FPG) and urine microalbumin (MALB) in patients with diabetic nephropathy. Methods The 106 patients diagnosed with diabetes in the hospital from July 2018 to January 2020 were selected as the research group and divided into nephropathy group (n=53) and non-nephropathy group (n=53) according to whether they had diabetic nephropathy. In the control group, 50 healthy persons were included in the hospital for physical examination during the same time period. Both groups of study participants were tested for HbA1c, FPG, and mALB, and the test results were compared between and within groups. Results The levels of HbA1c, FPG, and um-ALB of the study group were higher than those of the control group, and the difference was significant(P<0.05); the levels of HbA1c, FPG, and um-ALB of the patients in the nephropathy group were higher than those in the non-nephropathy group(P<0.05), it can be seen that its level was positively correlated with the severity of the disease; the positive rates of the three combined detection, HbA1c and FPG combined detection, and um-ALB single detection for diabetic nephropathy were respectively 83.02%, 66.04%, 24.53%, and the three combined detection positive rate was the highest(P<0.05). Conclusion HbA1c, FPG, and um-ALB levels have important clinical significance for the diagnosis of diabetic nephropathy and the judgment of disease severity. Combined detection can significantly improve the diagnostic accuracy, so it can be promoted and applied to the clinic., 百拇医药(林东海)
