STIC并TDI评价妊娠期糖尿病胎儿右心室功能价值(1)

     [摘要] 目的 探討时间-空间相关成像(STIC)技术联合组织多普勒成像(TDI)技术在妊娠期糖尿病(GDM)胎儿右心功能评价中的应用价值。方法 选取我院90例GDM胎儿为糖尿病组,同期另选90例正常妊娠胎儿为对照组,再根据妊娠时间分为3个亚组。运用STIC技术采集各组胎儿心动周期图像,获取右室舒张末期容积(RVEDV)、右室收缩末期容积(RVESV),计算出右室每搏心排血量(RVSV)、右室射血分数(RVEF)及右室心排血量(RVCO)。应用TDI技术分别测量三尖瓣舒张早期峰值(Ea)、舒张晚期峰值(Aa)和收缩期峰值(Sa),计算Ea/Aa值。对上述各指标结果进行比较分析。结果 对照组与糖尿病组右心室RVEDV、RVESV、RVSV、CO、Ea、Aa及Sa值均随孕周的增加而增大,差异均有统计学意义(F=5.31~9.88,r=0.53~0.74,P<0.05);而EF和Ea/Aa与孕周无相关性(P>0.05)。与对照组比较,32周龄后糖尿病组的右心室RVEDV、RVESV、RVSV、CO及EF增大,Sa值减小(F=3.85~6.56,P<0.05);24周龄后糖尿病组的Ea、Aa及Ea/Aa值均减小(F=4.13~5.41,P<0.05)。结论 STIC联合TDI可以全面客观地评价GDM胎儿右心室的收缩和舒张功能。

    [ABSTRACT] Objective To investigate the value of spatio-temporal image correlation (STIC) combined with tissue Doppler imaging (TDI) in the evaluation of fetal right fetal function in pregnant women with gestational diabetes mellitus (GDM). Me-thods Nighty fetuses with GDM in our hospital were selected as diabetes group, and another 90 normal pregnancy fetuses in the same period were selected as control group. They were divided into three subgroups according to the time of pregnancy. The fetal cardiac cycle images, right ventricular end-diastolic volume (RVEDV), right ventricular end-systolic volume (RVESV), right ventricular stroke volume (RVSV), right ventricular ejection fraction (RVEF), and right ventricular cardiac output (RVCO) were obtained using STIC technique. The early diastolic peak (Ea), late diastolic peak (Aa), and systolic peak (Sa) of tricuspid valve were measured by TDI technique, and the Ea/Aa values were calculated. The results of the above indicators were compared and analyzed. Results The values of RVEDV, RVESV, RVSV, CO, Ea, Aa, and Sa of the right ventricular in the control group and the diabetes group increased with the increasing gestational weeks (F=5.31-9.88,r=0.53-0.74,P<0.05), while EF and Ea/Aa had no correlation with gestational weeks (P>0.05). Compared with the control group, the diabetes group had significantly increased RVEDV, RVESV, RVSV, CO, and EF of the right ventricle and significantly reduced Sa after 32 weeks of gestation (F=3.85-6.56,P<0.05). Ea, Aa, and Ea/Aa in the diabetes group decreased after 24 weeks of gestation (F=4.13-5.41,P<0.05). Conclusion STIC combined with TDI can evaluate the systolic and diastolic function of the right ventricle in GDM fetus comprehensively and objectively., 百拇医药(杨丽玲 胡媛 姜志荣)
