200例糖尿病患者白内障摘除及后房型小切口无缝线人工晶体植入的临床分析(1)

[摘要] 目的：分析糖尿病患者白内障摘除及后房型小切口无缝线人工晶体植入术的临床疗效及并发症。方法：对200例(248只眼)糖尿病患者白内障摘除及后房型小切口无缝线人工晶体植入术的临床资料进行回顾性分析。结果：200例(248只眼)手术，术后平均随访18个月，术后视力0.3及以上者达187只眼，占75.40%，效果良好；术后并发症：晶体前囊色素、角膜纹状混浊、前房纤维素性渗出、虹膜后粘连、后囊混浊、房水混浊。结论：糖尿病患者血糖控制在8.0 mmol/L以下时，行后房型小切口无缝线人工晶体植入术是可行的。

Clinical analysis of 200 cases of diabetes in patients with cataract extraction and posterior chamber sutureless small incision intraocular lens implantation ZHANG Jing, WANG Wenqiang, SU Hongmei

[Abstract] Objective: To analyze the clinical effect and complications of diabetes and cataract extraction with posterior chamber sutureless small incision intraocular lens implantation operation. Methods: 200 patients (248 eyes) in patients with diabetic cataract extraction and posterior chamber sutureless small incision intraocular lens implantation clinical data were analyzed. Results: 200 cases(248 eyes) underwent surgery, patients were followed up for 18 months on average, 187 eyes had visual acuity of 0.3 or even greater than this value, accounting for 75.40%, and showed good effect; post-operative complications: crystal anterior capsule pigment, striated corneal opacity, anterior chamber exudative fibrin, iris rear adhesion, posterior capsular opacification, aqueous humor opacification. Conclusion: It is feasible for patients with diabetes to undergo posterior chamber sutureless small incision intraocular lens implantation when controlling blood sugar below 8.0 mmol/L.

随着生活条件的不断提高，我国糖尿病的患病率逐渐上升，糖尿病患者患老年性白内障较同年龄组非糖尿病者早，且发病率高。流行病学统计显示，糖尿病患者成熟期白内障的发生比普通老年性成熟期白内障早9.6年[1]。糖尿病性白内障已成为糖尿病并发症中仅次于视网膜病变的第二大眼病[2]。糖尿病患者白内障的治疗越来越被眼科工作者所重视，手术摘除是目前治疗的最有效可行的手段。我院1997年12月～2007年12月共完成200例(248只眼)糖尿病患者白内障摘除联合后房小切口无缝线人工晶体植入术，随访6～36个月，平均18个月。现将结果报道如下：
1 资料与方法
1.1 一般资料
我院1997年12月～2007年12月共收治200例(248只眼)伴有糖尿病的白内障患者，其中，单眼152例，双眼48例；男98例，女102例；年龄42～76岁，平均62岁。术前均经内科根据糖尿病诊断标准确诊为糖尿病，病史最长30年，最短3年，平均12年。全部为2型糖尿病病例，住院时血糖控制在8.0 mmol/L以下。全身检查心电图提示有冠心病67例，有高血压病史101例，术前血压均控制在140/90 mm Hg(1 mm Hg=0 ......
