急性一氧化碳中毒迟发性脑病防治分析(1)

     [摘要] 目的:探讨防治急性一氧化碳(CO)中毒迟发性脑病的有效方法。方法:将我院2007年1月~2008年12月收治的59例急性CO中毒患者随机分成治疗组30例,对照组29例,治疗组用高压氧加醒脑静治疗,对照组用高压氧治疗,治疗后对两组患者进行临床疗效分析。结果:治疗组的昏迷时间为(3.45±2.18) h,全部治愈,无迟发性脑病发生,脑电图及颅脑MRI均正常;对照组的昏迷时间为(4.64±2.12) h,治愈26例,迟发性脑病3例,其脑电图改变均为弥漫性慢波,颅脑MRI改变2例为皮层下脑电图质异常信号,1例为皮层下脑的质异常信号合并基底节区病变,两组对比临床疗效有显著性差异。结论:高压氧加醒脑静治疗急性CO中毒能更有效地防治迟发性脑病的发生,减少急性CO中毒的致残率,值得在临床推广应用。

    Analysis of prevention and treatment of acute carbon monoxide poisoning delayed encephalopathy

    [Abstract] Objective: To explore more effective therapy for acute carbon monoxide(CO) poisoning delayed encephalopathy.Methods: 59 cases of acute CO poisoning patients in our hospital from January 2007 to December 2008 were randomly divided into treatment group(30 cases)and control group(29 cases),treatment group treated with hyperbaric oxygen plus Xingnaojing treatment, control group with hyperbaric oxygen therapy. After treatment,clinical efficacy of two groups was analyzed. Results: Coma time of treatment group was (3.45±2.18) h; all cured and non-delayed encephalopathy occurred, EEG and brain MRI were normal.Coma time of control group was (4.64±2.12) h; 26 cases cured; 3 cases occureddelayed-encephalopathy, the EEG changes of which were diffuse slow-wave,2 cases of brain MRI changes, of which were the subcortical abnormal EEG signal quality and the other case was abnormal signal quality under the cerebral cortex and basal ganglia lesions.Clinical efficacy of two groups has significant difference compared with each other. Conclusion: Xingnaojing plusing hyperbaric oxygen therapy in the treatment of acute CO poisoning can more effectively control the occurrence of delayed encephalopathy, reduce acute CO poisoning disability.It is worthy of being applied in clinical.
