[摘要] 目的：比较采用不同剂量酒石酸布托啡诺联合甲磺酸罗哌卡因用于硬膜外分娩镇痛的疗效及安全性。方法：选择我院2009 年6 月～2010年6 月主动要求行分娩镇痛、健康、单胎、足月临产初产妇100例随机分成四组，A组采用0.002%酒石酸布托啡诺和0.178 8%甲磺酸罗哌卡因；B组采用 0.004%酒石酸布托啡诺和0.178 8%甲磺酸罗哌卡因；C组采用0.006%酒石酸布托啡诺和0.178 8%甲磺酸罗哌卡因；D组采用0.178 8%甲磺酸罗哌卡因。采用视觉模拟疼痛评分(VAS)，记录两组疼痛程度、疼痛开始缓解时间(T1)、疼痛完全缓解持续时间(T2)、产程时间、分娩方式、新生儿Apgar 评分及不良反应情况。结果：随着布托啡诺浓度的提高，T1逐渐缩短，与D 组比较，B、C组T1明显缩短。随着使用布托啡诺剂量的增加T2延长，与D组比较，B、C 组T2明显延长。结论：0.004%酒石酸布托啡诺和0.178 8%甲磺酸罗哌卡因用于分娩镇痛为最佳剂量且有效安全。

Effect exploration on different dosage of Butorphanol Tartrate in labour analgesia

[Abstract] Objective: To compare the efficacy and safety of Butorphanol Tartrate and Ropivacaine Mesylate in labour analgesia. Methods: From June 2009 to June 2010, 100 patients of epidural analgesia in labor, health, single fetal and the term of the provisional maternal were enrolled and divided randomly into four groups, A group with 0.002% Butorphanol Tartrate and 0.178 8% Ropivacaine Mesylate; B group with 0.004% Butorphanol Tartrate and 0.178 8% Ropivacaine Mesylate; C group with 0.006% Butorphanol Tartrate and 0.178 8% Ropivacaine Mesylate; D group with 0.178 8% Ropivacaine Mesylate. Then visual analogue scales (VAS), the initiating (T1) and lasting (T2) time of effectiveness, the time of labor course, the pain of childbirth, mode of delivery, motor nerve block and other indicators were observed and compared between two groups. Results: Butorphanol Tartrate decreased T1 and increased T2. T1 in B and C groups were shorter than that in D group. T2 in B and C groups were longer than that in D group. Conclusion: Butorphanol Tartrate can provide effective analgesia in labour analgesia with fewer side effects, especially when the dosage of 0.004% Butorphanol Tartrate and 0.178 8% ropivacaine mesylate was used.
