[摘要] 目的：探讨产钳术在处理第二产程头位难产中的重要作用及安全性，指导临床正确使用产钳术。方法：对我科2008年1月～2010年12月收集的产钳术90例进行回顾性分析。结果：低中位产钳术与低位产钳术、出口产钳术相比较，阴道撕裂伤、新生儿重度窒息方面差异有统计学意义(P<0.05)；会阴Ⅲ度裂伤、宫颈裂伤、产后出血、新生儿头皮血肿、颜面部擦伤、颜面部皮肤压迹、新生儿轻度窒息比较，差异无统计学意义(P>0.05)。阴道撕裂伤、阴道血肿、新生儿重度窒息与胎头位置高低有关，宫颈裂伤、产后出血、会阴Ⅲ度裂伤、新生儿头皮血肿、新生儿颜面部擦伤、新生儿颜面部压迹与产钳操作技术有关。结论：第二产程中正确使用产钳术能迅速结束分娩，中低位以下产钳，只要掌握适应证、规范操作，可降低母儿并发症发生率、降低剖宫产率。

Clinical analysis of the 90 cases of obstetric forceps operations

[Abstract] Objective: To investigate the security and role of application of obstetrics forceps in dystocia of vertex presentation during the second course of infant-production, to guide clinical use of the obstetrics forceps correctly. Methods: The data of 90 cases of obstetric forceps operations from January 2008 to December 2010 in our department of our hospital were analyzed. Results: The rates of vagina laceration and new born severe-asphyxia in mid-low forceps delivery were significantly higher than thaose in low forceps delivery and export position forceps (P<0.05). The three groups showed no significant difference in the rates of third-degree perineum laceration, cervix laceration, post-partum hemorrhage, new born scalp hematoma, new born facial abrasion or compression and new born mild-asphyxia (P>0.05). The rates of vagina laceration and newborns severe-asphyxia were related with the fetal head level. The rates of third-degree perineum laceration, cervix laceration, post-partum hemorrhage, new born scalp hematoma, new born facial abrasion or compression and new born mild-asphyxia were related to obstetric forceps technique. Conclusion: We can make a swift end to delivery by use the obstetric forceps correctly in the second course of infant-production. So we can reduce the rate of caesarean operation and the complication of mother and fetus.
