妊娠期糖尿病高危孕妇甲状腺功能及甲状腺自身抗体变化的研究(1)

[摘要] 目的：观察妊娠期糖尿病高危孕妇的甲状腺功能及自身抗体改变情况。方法：选取我院2008年10月～2010年10月收治的经临床判断为妊娠期糖尿病高危孕妇90例(高危组)，临床诊断为妊娠期糖尿病孕妇30例(糖尿病组)，正常健康孕妇40例(对照组)，检测三组孕妇T3、T4、FT3、FT4、TSH、TGAb指标，并进性比较分析。结果：糖尿病组及高危组孕妇甲状腺自身抗体阳性率(20.0%、18.8%)明显高于健康对照组(7.5%)，差异有统计学意义(χ2=13.47、12.19，P<0.05)；糖尿病组及高危组孕妇FT4水平[(5.40±1.08)pmol/L、(6.15±1.12)pmol/L]明显低于对照组[(7.59±1.53)pmol/L]，差异有统计学意义(t=4.29、3.17，P<0.05)，高危组及糖尿病组孕妇中同时有糖尿病及甲状腺疾病家族史的孕妇甲状腺自身抗体阳性率(50.0%、45.5%)明显高于无家族史(13.3%、8.2%)或仅有一种家族史的孕妇(22.2%、16.7%)。结论：妊娠期糖尿病孕妇及高危孕妇甲状腺功能异常的风险较高，尤其是同时有甲状腺及糖尿病家族史的孕妇，临床应早期进行甲状腺相关检查，避免甲状腺功能紊乱引起的妊娠期意外。

The study of thyroid autoantibodies and thyroid function changes in the pregnant women with high-risk of gestational diabetes mellitus

[Abstract] Objective: To evaluate the thyroid function and the prevalence of thyroid autoantibodies in the pregnant women with high-risk of gestational diabetes mellitus (GDM). Methods: 90 cases of the pregnant women with high-risk of GDM in our hospital from October 2008 to October 2010 were selected as high-risk group, 30 cases of pregnant women with GDM as diabetic group, and 40 cases as normal group, healthy pregnant women as control group; T3, T4, FT3, FT4, TSH, TGAb index of three groups were tested and analyzed.
