肠内营养对老年急性充血性心力衰竭患者的影响(1)

     [摘要] 目的:探讨肠内营养对老年急性充血性心力衰竭患者预后的影响。方法:收集2005年1月~2010年12月在我院老年科监护室住院的各种疾病所致的急性心力衰竭患者87例,在积极治疗原发病,合理氧疗,注意水、电解质、酸碱平衡,合理利用利尿剂、血管扩张剂及洋地黄制剂等治疗的基础上,将其随机分为肠内养组(EN组)43例及肠外静脉营养组(PN组)44例,观察患者14 d后血清白蛋白、血红蛋白、中心静脉压及生存例数。结果:经两种不同营养支持治疗14 d后,患者EN组与PN组血清白蛋白 [(36.0±3.2)g/L vs (34.0±2.5)g/L]、血红蛋白[(12.1±1.8)g/L vs (9.8±2.2)g/L],EN组与PN组比较均升高明显(P<0.05);中心静脉压(CVP)EN组较PN组[(12.1±2.5)cm H2O vs (13.5±3.2)cm H2O]下降(P<0.05);生存例数EN组(30例)较PN组(20例)增加明显(P<0.05)。结论:正确运用肠内营养支持有助于改善老年急性充血性心力衰竭患者预后,提高生存率,值得推广。

    Influence of enteral nutrition for the elderly patients with acute congested heart failure

    Department of Geriatrics, the First People's Hospital of Guangzhou City, Guangdong Province, Guangzhou 510180, China[Abstract] Objective: To investigate the influence of enteral nutrition for the elderly patients with acute congested heart failure on the prognosis. Methods: 87 inpatients due to acute congestive heart failure induced by all kinds of diseases from January 2005 to December 2010 in the old age branch guardianship room were randomly divided into two groups, group EN (enteral nutrition, 43 cases) and group PN (parenteral nutrition, 44 cases). Each group was actively treatment the primary affection, on original base of rational oxygen therapy, keeping water, electrolyte with balance, acid-alkali equilibrium, rational use of diuretic agent, vasodilator agent and digitalis. The serum albumin, hemoglobin, central venous pressure and the number of survival patients after 14 days were observed. Results: The serum albumin, group EN and group PN [(36.0±3.2) g/L vs (34.0±2.5) g/L], the hemoglobin, group EN and group PN [(12.1±1.8) g/L vs (9.8±2.2) g/L], after two different nutrition supports treatment 14 days, there were significant differences in the two groups (P<0.05); the central venous pressure (CVP) in group EN [(12.1±2.5) cm H2O] was drop than that in group PN [(13 ......
