TP方案联合放疗同步与序贯治疗中晚期宫颈癌的对比研究(1)

[摘要] 目的：探讨中晚期宫颈癌的治疗方法。方法：选取中晚期宫颈癌病例73例，分为同步治疗组(37例)和序贯治疗组(36例)。同步治疗组放疗开始给予紫杉醇135~175 mg/m2，静滴1 d；顺铂25~30 mg/m2，静滴1~3 d，放疗时间间隔28 d重复1次化疗。序贯治疗组给予紫杉醇+顺铂化疗，化疗间歇期行根治性放疗。结果：同步治疗组和序贯治疗组总有效率分别为89.2%(33/37)、69.4%(25/36)，两组比较，差异有统计学意义(P<0.05)。两组不良反应发生率比较，差异无统计学意义(P>0.05)。同步治疗组3年生存率为78.4%(29/37)，序贯治疗组为55.6%(20/36)，3年生存率同步治疗组高于序贯治疗组，差异有统计学意义(χ2=4.307，P<0.05)。结论：TP方案化疗联合放疗同步治疗中晚期宫颈癌的疗效高于序贯治疗组，值得在临床推广应用。

Contrast study on TP combined with radiotherapy concurrent and sequential therapy in treatment of advanced cervical cancer

[Abstract] Objective: To explore the method in the treatment of advanced cervical cancer. Methods: 73 advanced cervical patients were randomly divided into two groups: concurrent therapy group(37 cases) and sequential therapy group(36 cases). Concurrent therapy group was treated by PTX ,135-175 mg/m2, ivgtt, 1 d; DDP 25-30 mg/m2, ivgtt, 1-3 d, every 28 days after radiotherapy repeated chemotherapy once; Sequential therapy group was treated by PTX+DDP chemotherapy, in intermission of chemotherapy was given radiotherapy. Results: The total effective rate of the concurrent therapy group and the sequential therapy group were 89.2% (33/37) and 69.4% (25/36) respectively, there was a significant difference between them (P<0.05); There was no significant difference of adverse reaction (ADR) rates between the two groups (P>0.05); 3 year survival rate of the concurrent therapy group was 78.4% (29/37), it was more better that of sequential therapy group [55.6% (20/36)], there was a significant difference between them (χ2=4.307, P<0.05). Conclusion: TP combined with radiotherapy concurrent in treatment cervical cancer is better than sequential therapy group, and it is well worthy of clinical popularizing and application.
