单极电刀在颅骨修补手术中的应用(1)

     [摘要] 目的 探讨单极电刀在颅骨修补术中的应用。 方法 将2009年5月~2011年5月我科收集的颅骨缺损患者186例分为电刀组(98例)和手术刀组(88例),观察两组手术时间、术中出血量、术后24 h引流量以及术后并发皮瓣坏死、皮瓣感染、皮下积液等指标。 结果 电刀组手术时间与手术刀组比较,差异有高度统计学意义(P < 0.01),术中平均出血量电刀组较手术刀组显著减少(P < 0.01);电刀组术后24 h引流量较手术刀组显著减少(P < 0.01);电刀组和手术刀组术后皮瓣坏死、皮瓣感染、皮下积液方面比较,差异均有高度统计学意义(P < 0.01)。 结论 电刀在颅骨修补手术中的应用可使手术视野更加清晰,手术时间明显缩短,术中出血量显著减少,术后皮下积液和皮瓣坏死未见增多。单极电刀在颅骨修补术中操作简便、安全可靠。

    Application of monopolar electrotome in cranioplasty surgery

    [Abstract] Objective To discuss the application of monopolar electrotome in cranioplasty surgery. Methods 186 patients with skull defects from May 2009 to May 2011 in our department were divided into electrotome group (98 cases) and scalpel group (88 cases). And the operation time, blood loss, postoperative 24 h drainage, postoperative flap necrosis, flap infection, subcutaneous fluid and other indicators were observed. Results There was a significant difference between electrotome and scalpel group about the operation time (P <0.01), the mean amount of bleeding in electrotome group was significantly reduced compared with scalpel group (P < 0.01); the postoperative 24 h drainage in electrotome group was significantly reduced compared with scalpel group (P < 0.01); there were significant differences on the flap necrosis, flap infection and subcutaneous fluid between electrotome and scalpel group (P < 0.01). Conclusion Monopolar electrotome in skull repair surgery has a more clear operative field, and has shorter operative time, can significantly reduce blood loss, no increase of postoperative subcutaneous fluid and flap necrosis is found. Monopolar electrotome in cranioplasty surgery is simple, safe and reliable.
