全髋关节置换术与人工股骨头置换术治疗老年股骨颈骨折的临床对照观察(1)

[摘要] 目的 观察全髋关节置换术与人工股骨头置换术治疗老年股骨颈骨折的临床疗效，并关注治疗后患者的睡眠质量及生活质量的差别，以期为临床工作提供理论支持。 方法 收集我院168例老年股骨颈需进行假体置换术的患者，将其分为观察组(84例)与对照组(84例)。对照组行人工股骨头置换术，观察组行全髋关节置换术，观察两组患者手术时间、随访疗效评价、睡眠质量及生活质量的差别。 结果 观察组治疗后的优良率明显高于对照组(92.86% vs 82.14%)(χ2=4.408 2，P = 0.035 8)，观察组患者睡眠质量明显低于对照组[(6.46±2.46)分 vs (10.32±3.46)分](t =8.14，P = 0.013 2)，观察组患者生活质量明显高于对照组(均P < 0.05)，但观察组患者手术时间明显长于对照组[(108.43±13.25)min vs (70.25±23.13)min](t = 13.46，P < 0.01)。 结论 对老年股骨颈骨折的患者应积极进行全髋关节置换术，临床疗效满意，术后的睡眠及生活质量理想，临床中可以积极应用。

Clinical control observation on total hip replacement and artificial femoral head replacement in the treatment of elderly patients with femoral neck fracture

[Abstract] Objective To observe the clinical effect, sleep quality and life quality on patients with elderly femoral neck fracture, for directing clinical work. Methods 168 cases were divided into observation group (84 cases) and control group (84 cases). The control group was treated by artifical femeral head replacement. The observation groupwas treated by total hip replacement. The effect, sleep quality and life quality were observed in the two groups. Results The effect (92.86% vs 82.14%) (χ2=4.408 2, P = 0.035 8), sleep quality scores [(6.46±2.46) scores vs (10.32±3.46) scores] were better in the observation group than in the control group (t = 8.14, P = 0.013 2). The life quality scores were better in the observation group than in the control group (all P < 0.05). But the surgical time were longer in the observation group than in the control group [(108.43±13.25)min vs (70.25±23.13)min] (t = 13.46, P < 0.01). Conclusion The total hip replacement can improve the effect, increase sleep and life quality in patients with elderly femoral neck fracture, is worthy be commended in clinical treatment.
