鸦胆子油注射液对Ⅲ期胃癌的疗效及对机体免疫功能的影响(1)
摘要目的：探討鸦胆子油注射液对Ⅲ期胃癌的疗效，并观察其对患者免疫功能的影响。方法：选取2011年1月至2012年1月宝鸡解放军三医院收治的Ⅲ期胃癌患者40例，随机分为对照组和观察组，每组20例，2组胃癌患者均接受胃癌外科根治术，术后常规接受化疗治疗，观察组在上述基础治疗上加用鸦胆子注射液治疗，2组均接受为期6周的治疗，比较2组患者生命质量、生存率、不良反应的差别，同时比较2组免疫功能的变化。结果：1)2组治疗Karnofsky评分均升高，其中观察组升高的幅度高于对照组，差异有统计学意义，且观察组中位生存期明显高于对照组。2)治疗后2组CD3+CD4+、CD3+CD8+及CD3-CD16+CD56+水平均低于治疗前(t=425，P=0002)，其中对照组降低得更为明显，与观察组比差异有统计学意义(t=366，P=001)。且治疗后CD4+/CD8+较治疗前显著升高(t=273，P=0004)，对照组升高幅度更大(t=422，P=003)。3)2组患者均存在一定的不良反应，主要表现为白细胞减少、胃肠道反应、肝功能损害及脱发，其中2组胃肠道反应、肝功能损害及脱发发生率差异无统计学义意义(P>005)，而白细胞减少发生率2组比较差异有统计学意义(χ2=414，P<005)。结论：鸦胆子油注射液可通过提高Ⅲ期胃癌患者免疫功能而达到提高患者生命质量和生存率的目的。

Clinical Efficacy of Brucea Javanica Oil Injection in the Treatment of Stage Ⅲ Gastric Carcinoma and the Effects on the Immunological Function of the Organism

Both groups were treated for 6 weeks and the differences of the quality of life， survival rate， toxicity reaction as well as the changes of immune function in the two groups of patients were compared. Results： 1) The Karnofsky scores of the two groups both rose after treatment， and the rising range of the observation group was higher than the control group， and the differences were statistically significant. The median survival time of the observation group was obviously higher than that of the control group. 2) The level of CD3+CD4+、CD3+CD8+ and CD3CD16+CD56+ in the two groups after treatment were lower than before (P=0002，t=4.25)， and the control group lowered more obviously， and the difference was statistically significant compared with the observation group (P=001， t=3.66). And CD4+/CD8+ level significantly increased after treatment than before (P=0004， t=2.73)， the ascending range of the control group was greater (P=003，t=4.22). 3) Toxicity reactions existed in both groups and mainly showed leukopenia， gastrointestinal reaction， liver function damage and alopecia. The differences of gastrointestinal reaction， liver function damage and the incidence of alopecia of both groups had no statistical significance (P>005)， but the differences of the incidence of leukopenia between the two groups were statistically significant (chisquare=4.14， P<005). Conclusion： Brucea javanica oil injection can reach the purpose of improving the quality of life and survival rate of patients with stage Ⅲ gastric carcinoma by improving their immune function., http://www.100md.com(谭宝利 张进儒)
