针刀松解辅助治疗寒湿痹阻型肩周炎患者的近期疗效及机制(1)

     摘要目的：探讨针刀松解辅助治疗寒湿痹阻型肩周炎的近期疗效及机制。方法：选取2012年7月至2016年5月在濮阳市中医医院收治的寒湿痹阻型肩周炎患者118例，按照随机数表法分为对照组和观察组，每组59例。对照组接受常规康复性功能锻炼，观察组接受康复性功能锻炼加针刀松解辅助治疗。评估2组患者的近期疗效，对比血清中疼痛相关指标、炎性递质、血管功能的差异。结果：观察组的治疗总有效率高于对照组(P<005)；观察组治疗后血清中疼痛递质5羟色胺(5HT)、前列腺素E2(PGE2)、P物质(SP)的水平低于对照组，炎性递质白细胞介素6(IL6)、白细胞介素10(IL10)、肿瘤坏死因子α(TNFα)、中性粒细胞巨噬细胞集落刺激因子(GMCSF)的水平低于对照组，一氧化氮(NO)、一氧化氮合酶(NOS)低于对照组(P<005)。结论：寒湿痹阻型肩周炎接受针刀松解辅助治疗，在缓解病情方面具有肯定疗效，具体机制与其减轻患者炎性反应、优化血管功能等相关。

    Shortterm Effects of Acupuncture Relaxation on Colddampness Obstruction

    Type Scapulohumeral Periarthritis and Its Mechanism

    AbstractObjective：To study the shortterm effects of acupuncture relaxation on colddampness obstruction type scapulohumeral periarthritis and its mechanism. Methods：A total of 118 cases of colddampness obstruction type scapulohumeral periarthritis in our hospital from July 2012 to May 2016 were chosen as the research subject. All patients were divided into control group (n=59) and observation group (n=59) according to random number table. The patients in control group were treated with routine rehabilitation exercise， and those in observation group were treated with rehabilitation exercise and acupuncture relaxation treatment. Shortterm efficacy in two groups was evacuated and serum contents of pain medium， inflammatory mediators and vascular function indexes were compared. Results：The total efficiency in observation group was higher than that in control group (P<005). Serum contents of pain medium serum， such as 5HT， PGE2 and SP in observation group were lower than those in control group， and inflammatory mediators such as IL10， IL6， TNFα， GMCSF were lower than those in control group. Vascular function indexes such as NO and NOS were lower than those in control group (P<005). Conclusion：Acupuncture knife release adjuvant therapy in treating patients with colddampness obstruction type scapulohumeral periarthritis has certain curative effects in the remission of the disease， and the specific mechanism might be related with reducing inflammatory responses in patients and optimization of vascular function.
