活血化瘀法在治疗手部皲裂角化性湿疹中的应用(1)

【摘要】 目的 观察活血化瘀法治疗手部皲裂角化性湿疹的临床疗效。方法 将患者随机分为A、B两组，A组用自拟中药外洗浸泡双手，2次/d，同时于泡手后外用糠酸莫米松乳膏1次/d和尿素霜2~3次/d，B组外用糠酸莫米松乳膏1次/d和尿素霜2~3次/d，分别在治疗第7、14天和第28天观察其疗效，并在第28天治疗后对其临床有效率进行总结。结果 两组患者在治疗前和治疗第7天评分经统计学比较无明显差异(P>0.05)，但在治疗第14天和第28天时评分经统计学比较有显著性差异(P<0.01)。两组患者治疗后临床有效率经统计学比较有明显差异(Z=-4.555，P<0.01)且未发现有明显不适反应。结论 活血化瘀法能够明显提高对慢性皲裂角化性湿疹的疗效，且不良反应小，值得临床推广使用。

【Abstract】 Objective To observe the therapeutic effect of activating blood circulation to remove the stasis in treating keratinizing chapped hand eczema.Methods Patients were randomly divided into group A and group B.Patients in group A washed and soaked both hands using self-made traditional Chinese medicine twice a day at the same time，using mometasone furoate cream once a day and urea frost two to three times a day after washing hands.Patients in group B only used the mometasone furoate cream and urea frost.We observed the therapeutic effect on the 7th day，the 14th day and the 28th day，summarized the clinical effective rate on the 28th day after treating.Results The score of before treatment and treatment on the 7th day was no significant difference according to statistic analysis(P>0.05)between two groups.However，the score of treatment on the 14th day and the 28th day had statistically significant differences(P<0.01).The significant difference was shown statistically between two groups in the rate of effectiveness(Z=-4.555，P<0.01),and no obvious uncomfortable response was found.Conclusion The therapeutic effect can be improved markedly by using activating blood circulation to remove the stasis in treating keratinizing chapped hand eczema and with little side-effects.So it is deserved being generalized for clinical use.

手湿疹是皮肤科常见病之一，究其病因错综复杂，目前普遍认为外源性因素是其常见病因，并以刺激性病因为主，常因多种因素共同作用而发病，随着人们生活水平的不断提高，大量化学洗涤用品和消毒用品的应用越来越频繁，对手部皮肤的刺激增强，手部湿疹的发病率也相应增高，特别是女性患者尤为明显。笔者在2007年5月至2008年5月应用活血化瘀法对就诊于皮肤性病科门诊的手部皲裂性湿疹患者进行治疗，取得良好疗效，现将之进行总结。
1 临床资料
1.1 病例选择 所有病例均来自于2007年5月至2008年5月就诊于我科门诊的患者，且所有患者均有手部皮肤瘙痒、角化过度、伴有红斑、脱屑及皲裂等慢性湿疹表现。随机分为A、B两组。

1.2 排除标准 ①除手部皮损外全身皮肤尚有水疱、渗液、结痂等急性渗出性湿疹样损害者；②合并病毒、细菌、真菌、寄生虫等感染者；③未曾系统应用糖皮质激素和免疫抑制剂者；④停用与治疗本病有关的药物2周以上者；⑤患有严重糖尿病及心、肝、肾或血液等系统性疾病者；⑥妊娠或哺乳期女性。
1.3 一般资料 A组41例，其中男15例，女26例，年龄18~71岁，平均52.3岁，病程3.5~18年；B组42例，其中男13例，女29例，年龄20~69岁，平均55.1岁，病程4.3~21年。经统计学处理显示两组之间比较和构成比比较无显著性差异(P>0.05)。
1.4 治疗与评估方法
1.4.1 两组患者均外用0.1%糠酸莫米松乳膏(由上海先灵葆雅制药公司生产)，1次/d，及20%尿素霜(本院自制)，2~3次/d。
1.4.2 A组患者加用中药煎水浸泡手部皮损，每剂煎至1000 ml，放至温凉后泡手，2次/d，每次约40 min，自拟处方如下：当归30 g、红花30 g、丹参30 g、片姜黄20 g、桃仁20 g、蛇床子30 g、苦参30 g。
1.4.3 两组患者均以28 d为1个疗程，在开始治疗后第1、7、14、28天给予评分，以观察疗效。
1.4.4 观察项目评分标准(见表1)。
1.5 统计学方法 采用SPSS 10.0统计软件，计量资料采用均数±标准数(x±s)表示，采用重复测量以方差分析，等级资料采用非参数检验，计数资料采用χ2检验。, 百拇医药(张立新 杨顶权 宋佩华 白彦萍 尤立平)
