【摘要】 目的 观察瑞芬太尼复合异丙酚用于初孕无痛人工流产的临床效果,安全性,副作用等。方法 将120例要求行无痛人工流产的初孕妇随机分为两组:瑞芬太尼复合异丙酚组和异丙酚组,每组各60例。实验组静脉推注瑞芬太尼1.0 μg/ kg,在负荷剂量注完后同时均速静脉注射异丙酚1.5 mg/kg,术中必要时分次追加10~30 mg,以维持适当麻醉深度。对照组静脉异丙酚1.5 mg/kg,术中必要时分次追加10~30 mg,以维持适当麻醉深度。观察两组患者镇痛效果,宫口松弛情况,人工流产综合征发生率及苏醒效果。结果 与对照组相比,实验组镇痛和苏醒效果好、人工流产综合征发生率低;两组手术时间、术中出血比较,差异无显著性(P>0.05)。结论 瑞芬太尼复合异丙酚可为门诊无痛人工流产术患者提供满意的麻醉镇痛,术中患者血压、心率平稳,苏醒彻底,不良反应少。

【Abstract】 Objective To observe the Remifentanil combined with propofol for induced abortion in early pregnancy the clinical effects,safety,side effects.Methods 120 cases of induced abortion pregnant women were randomly divided into two groups:experimental group and control group,each group 60 cases.Experimental group,intravenous injection of remifentanil 1.0 μg/kg,injected propofol 1.5 mg/kg after the loading dose injection; the control group 1.5 mg/kg,both groups during times when necessary an additional 10~30 mg,in order to maintain an appropriate depth of anesthesia.analgesic effects,Miyaguchi relaxation,the incidence of induced abortion syndrome and the effect of waking up were observed.Results Compared with control group,experimental group the effect of analgesia and regained consciousness,and the incidence of induced abortion syndrome low; there were no significant difference between the operative time,intraoperative blood loss of the two groups.Conclusion Remifentanil combined with propofol may be painless outpatient abortion for patients to provide satisfactory analgesia anesthesia,intraoperative blood pressure,heart rate steady,thoroughly awake,and less side effects.

人工流产术作为避孕失败的补救措施,是计划生育门诊最常见的手术。随着人们生活质量的提高,对人工流产术中的无痛需求越来越高。目前,异丙酚静脉缓慢注射广泛用于人工流产术中麻醉镇痛,但是异丙酚的镇静作用强而镇痛作用相对较弱,致使部分患者效果仍不太满意[1]。瑞芬太尼是强有效镇痛药,我科从2003~2006年,采用瑞芬太尼复合异丙酚麻醉下行人工流产术,取得良好麻醉效果,现进行总结如下。
1 资料和方法
1.1 临床资料 临床资料选择2005年6月至2006年4月在我院妇科门诊自愿要求行无痛人流手术的早孕妇女120例,年龄18~42岁,停经42~75 d,均经腹部B超诊断为宫内妊娠。其中有剖宫产史55例,阴道分娩史30例,未生育者35例。所有患者无药物过敏史,无生殖系统慢性炎症及心血管病史,常规行血系列及出凝血时间检查 ......
