【摘要】 目的 探讨吻合口平面对结直肠术后吻合口漏发生率的影响。方法 选择2000年3月至2009年3月在我院行结直肠手术患者456例,根据结直肠手术不同的吻合平面分为3组,A组吻合口平面在横结肠;B组吻合口平面在乙状结肠或者直肠上段;C组为直肠中下段。记录每组手术例数和发生吻合口漏例数,根据以上划分标准分析结直肠手术不同吻合平面对结直肠吻合口漏的影响。结果 A组、B组和C组吻合口漏发生率经卡方检验,差异有统计学意义,P<0.05;B组吻合口漏发生率与A组吻合口漏发生率经卡方检验,差异有统计学意义(P<0.05);C组吻合口漏发生率与A组吻合口漏发生率经卡方检验,差异有统计学意义(P<0.05);B组吻合口漏发生率与C组吻合口漏发生率经卡方检验,差异无统计学意义(P<0.05)。结论 对于结直肠手术吻合口漏来说,其发生率与吻合口平面高低有密切关系,吻合口平面越低,吻合口漏的发生率越高。

Clinical analysis of anastomotic plane for the incidence of colorectal anastomotic leakage in colorectal surgery

【Abstract】 Objective To explore the effect of anastomotic plane for colorectal anastomotic leakage in colorectal surgery.Methods 456 cases with colorectal surgery were selected from March,2003 to March,2009.according to the different anastomotic plane in colorectal surgery,all the cases were three groups,Agroup(anastomotic plane was at transverse colon),Bgroup(anastomotic plane was at sigmoid or the upper segment of rectum),C group(anastomotic plane was at the middle and distal of rectum).according to the above standard,the effect of different anastomotic plane for colorectal anastomotic leakage in colorectal surgery was analyzed.Results There was statistical difference for anastomotic leakage in A、B and C group through chi-square test(P<0.05);there was statistical difference for anastomotic leakage in A and B group through chi-square test(P<0.05);there was statistical difference for anastomotic leakage in A andC group through chi-square test(P<0.05);there was no statistical difference for anastomotic leakage in C andB group through chi-square test(P<0.05).Conclusion The incidence of anastomotic leakage in colorectal surgery relates with the anastomotic plane,and the anastomotic plane is more lower,the the incidence of anastomotic leakage is more higher.

结直肠术后发生的吻合口漏是结直肠手术严重的并发症之一。吻合口漏的发生表明肠道手术失败,不但增加了患者痛苦还使后续治疗难度增加,再者吻合口漏还是吻合口狭窄的常见原因,这些都为给患者的生活质量带来影响,影响了患者的存活时间[1] ......
