PPH联合外痔切剥术与外剥内扎术治疗混合痔的对比研究(1)

【摘要】 目的 研究PPH联合外痔切剥术对混合痔的疗效。方法 将60例患者随机分为治疗组与对照组，治疗组予以PPH联合外痔切剥术,对照组予以外剥内扎术。观察两组手术一般情况以及术后并发症比较。结果 治疗组疼痛时间、康复时间较对照组短，两组比较差异具有统计学意义(P<0.05)，术后2 d疼痛程度积分、术后36 h麻醉药物使用次数比较治疗组较对照组水平低，两组比较差异具有统计学意义(P<0.05)。治疗组与对照组尿潴留率、排便困难率差异无统计学意义(P>0.05)，治疗组与对照组急便感、便时出血比较均差异有统计学意义(P<0.05)，1年复发率比较两组差异无统计学意义(P>0.05)。结论 PPH联合外痔切剥术的创伤性小于传统手术的损伤，还可降低PPH术后的复发率和并发症的发生率，减轻术后不适。

The comparative study of PPH combined with external hemorrhoids cutting and stripping and nluigah treatment of mixed hemorrhoid

【Abstract】 Objective
To study the effect of PPH combined with exterral hemorrhoids cutting and strpping and milligan treatment ofmixed hemorrhoid.Methods 60 patients were randomly divided into treatment group and control group, treatment group was treated with combined external hemorrhoids cutting and stripping PPH patients in the control group was treated with external dissection and internal ligation. Observed two groups of surgery and postoperative complications in general, comparison of the complications. ResultsThe time of pain, shorter recovery time than the control group, the difference was significant (P<0.05), postoperative pain points 2 days, 36 hours after the number of narcotic drug use compared the level of the treatment group than the control group low, the difference was significant (P<0.05). Treatment group and control group retention rates, difficult defecation rate did not differ significantly (P> 0.05), treatment and control groups anxious they feel, they bleed when comparison were significant differences (P<0.05), 1-year recurrence rate was relatively The difference was not significant (P> 0.05). ConclusionPPH combined external hemorrhoids cutting and stripping surgery than conventional surgery, traumatic injury, can reduce the recurrence rate after reducing PPH and the incidence of complications, less postoperative discomfort.

近年来由于PPH术较传统外剥内扎书具有术后疼痛轻、恢复快优点，成为重度内痔的常规手术之一，但并不适用于外痔治疗。所以，有很多外科医师主张在PPH的同时切除外痔[1]。为此本院自2006年对混合痔采用PPH联合外痔切除术与传统外剥内扎术的对比研究。
