中西医联合治疗放射性肺炎的疗效及其机制(1)

     【摘要】目的观察中西医结合疗法对RP的临床疗效及其机制。方法 RP患者63例随机分为西药组及中西医联合治疗组。西药组予以抗生素及激素治疗，联合治疗组在西药组基础上加用中药汤剂；治疗前后测定患者血浆IL1α，IL6，TNFα水平。结果 ①联合治疗组有效率显著高于西药组(P<0.05)。②RP患者血浆IL1α，IL6，TNFα水平显著高于正常对照组(P<0.05)；治疗后其水平较治疗前显著降低(P<0.05)，且治疗后联合治疗组显著低于西药组(P<0.05)。结论中西医联合疗法对RP有良好的治疗作用，其部分机制可能与调节IL1α，IL6，TNFα水平有关。

    【Abstract】 Objective

    To study the therapeutic effect and mechanism of combination therapy of Chinese and western medicine on radiation pneumonitis.Methods Sixtythree patients with radiation pneumonitis were randomly divided into western medicine group and combination therapy group. The western medicine group was given antibiotic and dexamethasone, the combination therapy group was given antibiotic, dexamethasone and traditional Chinese medicine. Levels of IL1α, IL6，and TNFα in plasma were measured by ELISA before and after the treatment. Results ① The therapeutic effect of the combination therapy group was better than the western medicine group (P<0.05). ② Levels of IL1α, IL6，and TNFα in patients with radiation pneumonitis were higher than in the healthy people(P<0.05). The levels significantly decreased after the treatment (P<0.05), and decreased even more in the combination therapy group than in the western medicine group (P<0.05).

    Conclusion The traditional Chinese medicine could exert its therapeutic effect on radiation pneumonitis by decreasing the levels of IL1α, IL6，and TNFα.

    1 资料与方法

    1.1 一般资料

    选择2009年1月至2010年12月门诊及病房RP患者63例，随机分为西药组及中西医联合治疗组。联合治疗组33例，其中肺癌15例，乳腺癌8例，食管癌10例；西药组30例，其中肺癌15例，乳腺癌7例，食管癌8例。纳入同期门诊健康查体人员10例，作为正常对照组。

    1.2 纳入标准

    患者正在或者近期肺部接受过一定放射量的放疗；有发热、咳嗽、咳痰、胸痛及呼吸困难等临床表现；胸片、胸部CT示显示肺内的病变范围与放射野一致；排除局部感染和复发；血常规：白细胞可正常或偏高 ......
