医院抗菌药物不合理用药情况分析(1)

【摘要】 目的 分析并探讨抗菌药物的不合理用药现状，为制定相应对策提供参考。方法 对所抽查的全部处方以及相关资料进行分类整理，主要对抗菌药物处方的选用、给药方案、用法用量、联合用药、药理拮抗等情况进行分类统计。结果 本组研究共抽查覆盖各个科室的处方6290张，其中有2140张处方为抗菌药物处方，占总抽查数的34.02%。对2140张抗菌药物处方进行检查发现不合理用药处方共有584张，占27.29%，并且一部分抗菌药物不合理处方中有多项不合理用药情况。对不合理用药情况进行分析表明不合理用药类别主要为：选择不合理、给药方案不合理、重复给药、无指征滥用、理化禁忌、毒性相加以及药理拮抗等。结论 医院药剂科必须对门诊处方以及住院处方进行严格检查与监督，并制定有效的管理制度及措施，定期组织开展抗菌药物用药知识的相关培训，及时纠正抗菌药物的不合理用药现象，提高抗菌药物的合理用药水平及医疗质量。

【Abstract】 Objective
To analyze and explore the status of irrational use of antibiotics, to provide a reference for the development of appropriate countermeasures. Methods The sampling for all prescriptions and related information to sort out, mainly on the choice of antibiotic prescriptions, dosage regimen, dosage, combination therapy, pharmacological antagonism, etc.Results The study covered all sections of the total sample prescription 6290, of which 2140 prescriptions for the antibiotic prescription, 34.02% of the total number of checks. Antibiotic prescriptions for 2140 inspection found that a total of 584 irrational prescriptions, accounting for 27.29%, and irrational prescription of antibiotics as part of a number of irrational drug use. Analyzed of irrational drug use that type of irrational drug use mainly: select unreasonable, irrational administration of the program, repeated administration, no indications of abuse, physical and chemical taboo, additive toxicity and other pharmacological antagonist. Conclusion Hospital pharmacy and hospital outpatient prescriptions must be strict inspection and monitoring prescription, and to develop effective management systems and practices, regularly organized anti-bacterial drugs in the knowledge of the relevant training, and promptly correct the phenomenon of irrational use of antibiotics, improved antibiotics the level of rational use of drugs and medical quality.

如何解决不合理用药问题在我国乃至其他国家都是一个亟待解决的问题，特别是抗菌药物的不合理使用现象最为普遍，例如，无适应证用药、选药及给药途径不合理、无指征用药、围手术期给药时间不当以及换药频繁等现象时有发生，这些不合理用药现象都是不良事件发生的隐患。为使抗菌药物能够得到合理使用，对我院在2010年9月至2011年2月期间的门诊处方和住院处方随机进行抽查，同时对抗菌药物的不合理使用状况进行统计分析并将结果及时反馈给各科室，旨在使抗菌药物能够合理使用，提高我院医疗服务质量。
1 资料与方法
1.1 一般资料
对我院在2010年9月至2011年2月期间的门诊处方和住院处方随机进行抽查，共抽取6290张。所抽查的处方样本能够覆盖本院全部科室。
