     【摘要】目的探讨异维A酸对痤疮患者外周血白细胞介素6(Interleukin6, IL6)的影响。方法选择重度痤疮患者30例，口服异维A酸胶丸治疗，10 mg/次，2~3次/d，12周为一疗程，采用酶联免疫吸附试验(ELISA)检测患者治疗前后血清中IL6的水平，另外选择30名健康体检者作为正常对照。结果治疗前重度痤疮患者血清中IL6的水平明显高于正常对照者(t=7.75, P<0.01)；治疗后重度痤疮患者血清中IL6的水平较治疗前明显降低(t=6.78, P<0.01)；治疗后重度痤疮患者血清中IL6的水平与正常对照组相比，差异无统计学意义(t=1.88, P= 0.0653)。结论异维A酸能抑制痤疮患者外周血IL6的过度表达，这可能是异维A酸治疗痤疮的作用机制之一。

    Inhibition of isotretinoin capsules on interleukin 6 in the treatment of acne

    【Abstract】 Objective

    To investigate the effect on interleukin 6(IL6) in the treatment of severe acne by isotretinoin capsules. Methods Thirty adult patients with acne and 30 health control were sequentially enrolled in this study. The patients were treated with isotretinoin capsules(10 mg,bid to tid) for 12 weeks. Expression level of IL6 was decected by Elisa methods. Results The serum level of IL6 in severe acne patients was statistically significantly higher than that of healthy control group (t=7.75, P<0.001). The serum level of IL6 in severe acne patients after treatment group was statistically significantly higher than that of before treatment group(t=6.78, P<0.001). The serum level of IL6 in after treatment group was not significantly different with healthy control group(t=1.88, P= 0.0653). Conclusion Our results show that isotretinoin could inhabit the over expression of IL6.These changes could be part of the therapeutic effects of isotretinoin on acne.

    痤疮是一种常见的皮肤附属器疾病，重度痤疮以脓疱、结节以及囊肿为主要表现，愈合形成萎缩或增生性瘢痕，严重影响患者面容[1]。异维A酸治疗重度痤疮的疗效显著[2]，但关于异维A酸作用机制尚未完全清楚。本研究选取前炎症因子IL6作为观察的指标，探讨异维A酸对重度痤疮患者外周血IL6表达的影响，现将结果报告如下。

    1 资料与方法

    1.1 一般资料收集30例重度痤疮门诊患者，其中男18例，女12例；年龄18～33岁，平均23.2岁；病程1个月至3年。重度痤疮诊断标准参考文献[2]:采用国际改良分级方法中的Ⅳ级标准，临床表现为粉刺，伴有大量丘疹和脓疱，分布广泛，总病灶数在100个以上，同时伴有结节与囊肿及疼痛。患者纳入标准：符合上述诊断标准，就诊前4周没有系统使用过痤疮药物。排除标准： ①合并有心脑血管、肝、肾、内分泌系统及造血系统等严重原发性疾病，精神病患者。②近一年之内有计划生育者。③妊娠或哺乳期妇女。④过敏体质或对本药物成分过敏者。另外选择体检中心30名非痤疮的健康体健者作为测定指标的正常参考值，其中男20例，女10例；年龄18～45岁，平均25.4岁。两组的年龄和性别经统计学处理差异无统计学意义(P>0.05),具有可比性。

    1.2 血清标本采血时间安排在上午空腹时，女性避开在月经期取血做检查。待测血清均存于20℃备用。
