【摘要】 目的 比较地佐辛或芬太尼复合丙泊酚静脉麻醉用于人工流产术的临床效果及安全性。方法 自愿要求行人工流产者90例,ASA ⅠⅡ级,随机分为3组(n=30), 1组先缓慢静脉注射地佐辛5 mg, 10 min后静脉注射丙泊酚2 mg/kg; 11组先缓慢静脉注射芬太尼1 μg/kg,10 min后静脉注射丙泊酚2 mg/kg;Ⅲ组先缓慢静脉注射地佐辛5 mg,8 min后静脉注射芬太尼0.5 w岁kg, 10 min后静脉注射丙泊酚2 mg/kg。观察各组MAP, HR,RR,SpO2,意识消失及苏醒时间、丙泊酚用药总量、麻醉效果、不良反应及术后疼痛评分。结果 3组苏醒时间、丙泊酚用量、麻醉与镇痛效果差异无统计学意义(P>0.05);3组MAP、SpO2均低于用药前(P<0.05); Ⅱ组用药后SpO2低于I、Ⅲ组(P<0.05); I组术中术后不良反应发生率低于Ⅱ、Ⅲ组(P<0.05); Ⅱ组术后下腹部疼痛VAS评分0~1分者少于I、Ⅲ组(P<0.05)。结论 地佐辛用于人工流产手术镇痛效果佳,麻醉效果满意,与芬太尼比较不增加丙泊酚用量,呼吸抑制等不良反应减少,单独或与小剂量芬太尼合用术后下腹痛发生率降低。

【Abstract】 Objective
Comparison of dezocine or fentanyl combined with propofol intravenous anesthesia in artificial abortion of clinical effect and safety. Methods Nighty ASA Ⅰ~Ⅱinduced abortion patients were randomly divided into 3 groups (n=30),Groups Ⅰwas administrated with 5 mg dezocine first, after 10 min with 2 mg/kg propofol,Groups Ⅱ was administrated with 1 μg/kg fentanyl first, after 10 min with 2 mg/kg propofol,Group Ⅲ was administrated with 5 mg dezocine first, after 8 min with 0.5 μg/kg fentanyl, after 10 min with 2 mg/kg propofol. Observed in MAP, HR, SpO2, RR, loss of consciousness and awakening time, Propofol dose, anesthetic effect, adverse reaction and the postoperative pain score. Results All Group was no significant difference in recovery time, the dosage of propofol, the anesthetic effect(P>0.05).All group MAP, SPO were lower than those before treatment (P<0.05),Group II after administration of SpO2 less than I、Ⅲ group (P< 0.05).Group I intraoperative and postoperative adverse reaction the incidence is lower than II、Ⅲ Group (P< 0.05),Group Ⅱ after lower abdominal pain VAS score 0 a 1 points less than I、Ⅲ group(P<0.05). Conclusion Dezocine have analgesic effect and satisfactory effect for artificial abortion operation anesthesia, compared with fentanyl, do not increase the dosage of propofol,respiratory depression and other adverse reaction is lower, alone or with small dose of fentanyl, reduction of the incidence of postoperative abdominal pain ......
