【摘要】 目的 探讨小剂量氯胺酮复合异丙酚静脉麻醉在经十二指肠胆道检查治疗技术中的安全性与可行性。方法 将80例患者随机均分成两组。麻醉组在异丙酚和氯胺酮复合麻醉下进行检查和治疗;对照组术前肌肉注射哌替啶50 mg和地西泮10 mg。观察比较两组患者的呼吸循环指标、操作过程评分和镇静评分。结果 麻醉组操作评分和镇静评分明显优于对照组,两组术前循环指标无统计学差异,麻醉组术中循环指标与术前比较无统计学差异,但显著性低于常规组的相同时间点(P<0.05);常规组术中循环指标显著性高于其术前(P<0.05)。结论 小剂量氯胺酮复合异丙酚的麻醉方法应用于经十二指肠胆道检查治疗术是安全可行的,并且该方法对循环指标不稳定和依从性差的患者具有优越性。

Application of small dose Katemine and propofol during ERCP

【Abstract】 Objective To evaluate the safety and feasibility of vein anesthesia using propofol combined with katemine on patients performing endoscopic retrograde cholangiopancreatography(ERCP).Methods 60 patients were randomly divided into two groups.The patients in anesthesia group received vein anesthesia using proppfol combined with katemine during ERCP.The patients in control group received preoperative intramuscular injections of pethidine 50 mg and diazepam 10 mg.The respiratory and circulation indices,performing process score and sedation score were observed and compared between the two groups.Results The performing process score and sedation score in anesthesia group were significantly better than those in control group(<0.05).The preoperative circulation indices between the two groups were similar.The circulation indices between pre and during operation in anesthesia group were similar too.But the circulation indices during operation in anesthesia group were significantly lower than those in control group(P<0.05).The circulation indices during operation in control group were significantly higher than those before operation(P<0.05).Conclusion The vein anesthesia using small done katemine combined with propofol on patients performing endoscopic retrograde cholangiopancreatography(ERCP)is safe and feasible,which is better for patients with unstability of circulation and poor compliance.
