【摘要】 目的 探讨本地区消化性溃疡应用不同治疗方案成本效果与生命质量的关系。方法 选取幽门螺杆菌阳性(Helicobacter pylori,Hp)的消化性溃疡患者165例,随机分成三组,A组:55例(奥美拉唑胶囊+阿莫西林胶囊+呋喃唑酮片),B组:55例(埃索镁拉唑肠溶片+阿莫西林胶囊+克拉霉素片),C组:(枸橼酸铋钾胶囊+阿莫西林胶囊+甲硝唑片),三组疗程均为四周,观察Hp根除率及溃疡总有效率,运用药物经济学的成本-效果分析对三种方案进行评价;三组患者均采用消化性溃疡患者生命质量测量表QLICD-PU,分别在治疗前、治疗后3个月、6个月进行生命质量评价。结果 ①三种治疗方案的成本效果比较,A组与B组比较差异无统计学意义,但在C组治疗方案的基础上,要多获得一个单位的疗效,B组增加的成本比A组明显减少;而C组与A组、B组比较,差异均有统计学意义(P<0.05);②治疗后三组患者生命质量总得分比较,A组与B组比较差异无统计学意义,C组与A组、B组比较差异均有统计学意义(P<0.05)。结论 通过对三组患者成本效果与生命质量研究,B组治疗方案最佳,溃疡的愈合率高,能更好的提高患者的生命质量,是一种既经济有效又能提高消化性溃疡生命质量的治疗方案。

Study of cost-effectiveness analysis(CEA) and quality of life on different treatment protocols for peptic ulcer

【Abstract】 Objective To study the cost-effectiveness and quality of life on different treatment protocols for patients with peptic ulcer . Methods 165patients of helicobacter pylori positive peptic ulcer were divided into three groups randomly, Group A: 55 cases (Omeprazole Capsules + Amoxycillin Capsules + Furazolidone Tablets). Group B: 55 cases (Esomeprazole + Amoxycillin Capsules + Clarithromycin Tablets). Group C: 55 cases (Bismuth Postassium Citrate Capsules + Amoxycillin Capsules + Metronidazole Tablets). The treatment course of each group was four weeks. We studied the HP eradication and response rate of ulcer, evaluated three treatment protocols bycost-effectiveness analysis of pharmacoeconomics and appraised patients’ quality of life through QLICD-PU before and after three 、six months treatment separately. Results The cost-effectiveness comparison of three groups indicated that the comparative difference of Group A and B had no statistical significance, but the two groups had one more unit of curative effect than Group C. The cost of Group B was obviously less than that of Group A. While Group C compared with Group A and B, the difference had obvious statistical significance (P<0.05).
