高频振荡通气叠加常频通气治疗新生儿重症气胸的临床疗效研究

     [摘要] 目的 探讨高频振荡通气叠加常频通气治疗新生儿气胸的临床疗效 。方法 选取2006年1月~2011年1月我院收治的重症气胸并呼吸衰竭新生儿49例,均进行胸腔闭式引流。25例高频振荡通气(HFOV)治疗设为对照组 ,24例高频振荡通气叠加常频通气(HFOV+SIMV)治疗设为治疗组 ,观察两组患儿2 、8 、12 、24h吸入氧浓度、动脉血气氧分压、二氧化碳分压、氧合指数以及胸片恢复时间、呼吸机使用时间、住院天数、住院费用。结果 治疗不同时间,治疗组吸入氧浓度、动脉血气氧分压、二氧化碳分压、氧合指数、动脉/肺泡氧分压比值与对照组相比差异有显著性(P<0.05)。治疗组胸片恢复时间、呼吸机使用时间、住院天数、住院费用优于对照组,差异有显著意义(P<0.05)。结论 高频振荡通气叠加常频通气治疗新生儿重症气胸较单纯高频振荡通气治疗疗效显著。

    Combination Effects of High Frequency Oscillatory Ventilation and Conventional Mandatory Ventilation in Treatment of Newborns with Intensive Pneumothorax

    [Abstact] Objective To evaluate the combination efects of high frequency oscillatory wentilation and conventional mandatory wentilation in treatment of newborns with intensive pneumothorax. Methods A total of 49 newborns with intensive pneumothorax in our hospital from January 2006 to January 2011,which were on the closed drainage of pleural cavity. They were divided into two groups:the control group(HFOV)and the research group (HFOV+SIMV)according the mechanical ventilation mode.Then we survey their fraction of inspired oxygen (FiO2)(after 2、8、12、and 24hours), arterial oxygen staturation(PaO2),arterial partial pressyre of carton dioxide(PaCO2),oxygenation index(OI),the recovery time of chest film,the use time of respirator,the hospital stays,the hospitalization expense. Result If the time of therapy was different,the difference was significant among the two groups about FIO.PaO2,PaCO2,OI, arterial/alveolar oxygen tension ratio(a/APO2).The treament group was better than the control group(P<0.05),on the recovery time of chest film,the use time of respirator ,the hospital stays,the hospitalization expense,so the difference was significant(P<0.05).Conclusion The combination of HFOV and SIMV was superior to conventional mandatory ventilation in treatment of newborns with intensive pneumothorax ......
