卒中单元超早期康复治疗对偏瘫患者的价值分析(1)

[摘要] 目的 探讨卒中单元超早期康复治疗对偏瘫患者运动功能恢复的影响。方法 2009年6月～2011年2月入住我科的94例住院患者随机分为卒中单元康复组与普通病房对照组。普通病房对照组患者在各项生命体征平稳后开始进行康复治疗；而卒中单元康复组患者按照卒中单元运作要求的模式进行超早期规范化康复治疗。观察两组患者治疗前、治疗后2周和8周的日常生活活动能力(ADL)评价、患肢运动功能评价以及两组患者的治疗效果。结果 治疗2周和8周后，卒中单元康复组患者的Bartel指数及Fugl-Meyer值较普通病房对照组同一时间明显升高(P<0.05)；且治疗后卒中单元康复组总有效率较普通病房对照组明显升高(P<0.01)。结论 卒中单元超早期康复治疗可有效改善偏瘫患者的运动功能，避免出现废用综合征，提高患者生活质量。

The Value of Ultra-early Stroke Unit Rehabilitation in Patients with Hemiplegia

[Abstract] Objective To explore the effective of stroke unit of super early rehabilitation therapy for stroke patients with hemiplegia. Methods All 94 patients who checked in our hospital during June 2009 to February 2011 were randomly divided into stroke units group and control group. The control group were given rehabilitation therapy when the patients’vital signs were smooth，but the therapy was not the routine. Stroke units group were given super early rehabilitation therapy according to stroke unit operation requirements. Before and after treatment，to observe two groups’ activities of daily life ability(ADL)evaluation and limb movement function evaluation，and the therapeutic effect of two groups. Results After treatment，the Bartel index and Fugl - Meyer value of stroke unit were higher than control group(P<0.05). And the total effectiveness of stroke unit were higher than control group(P<0.01). Conclusion Stroke unit of super early rehabilitation can improve the movement function of patients with hemiplegia. It can avoid waste with syndrome，and improve the quality of life of the patients.

脑卒中在老年人中是一种常见病、多发病，在我国，脑卒中导致的病死率仅次于癌症，中青年发病率有明显上升趋势，其致残率仍高达80%，如何有效治疗脑卒中，是临床医师在来断研究和探讨的问题。目前有研究证明卒中单元(stroke unit，SU)是治疗脑卒中最有效的方法[1]，其强调早期规范化康复治疗。近几年来，我院在一般药物治疗的基础上，实行卒中单元超早期规范化康复治疗，收到良好的效果，现报道如下。
1 资料与方法
1.1 一般资料
选择2009年6月～2011年2月在我科住院患者共94例，符合第四届全国脑血管病的诊断标准[2]。并经头颅CT或MRI确诊为脑梗死或脑出血者。均第一次发病，脑梗死发病后1～3d，脑出血发病后10～14d，偏瘫肢体肌力≤3级，Glasgow昏迷量表评分>8分 ......
