锁定钢板和传统钢板固定老年肱骨近端骨折的临床研究(2)

本组资料可见，锁定钢板治疗肱骨近端骨折明显要优于以往使用的传统钢板，不管是手术的创伤还是术后的恢复，锁定钢板的治疗效果要好很多，可能与跟锁定钢板的解剖设计、锁定成角稳定性有关。尽管锁定钢板具有创伤小，骨膜及软组织剥离少等特点，但本组中仍发生2例肱骨头缺血坏死，究其原因，可能由于①此2例患者均为四部分骨折；②骨质疏松严重；③骨折线延伸到关节囊内；④骨折移位较大；这些原因所致，所以对于AO/ASIF分型之C型骨折是关节囊内解剖颈骨折，建议行人工肩关节置换手术[9]。
综上所述，老年性肱骨近端骨折伴有骨折疏松，且骨折移位较大治疗的选择仍是一个比较棘手的问题，对于此类复杂的骨折，锁定钢板为我们提供了一种新的治疗方法，且是一种比较理想的术式。
